Anal Fissure/ Hemorrhoid?

Can anybody please shed any light on what might be causing my constant discomfort which is making me miserable.

For the last few weeks I have been having some terrible burning around the anus mainly at night but also after a BM. It all started after a week of constipation which caused some blood on the tissue. I had a very similar experience two years ago which lasted around 6 weeks.

The most significant symptom is the anal spasms that causing an annoying burning sensation around the anus, it feels like the anus is cramping/tightening. This is much worse after a BM and the burning can last 3/4 hours which makes it very uncomfortable to sit.

The anus can sometimes feel moist and passing wind can result in burning 10/15 minutes later. There has been some feeling of fullness which comes and goes. I have had a number of rectal exams over the years by a number of Doctors usually after symptoms have subsided but they have always stated that Internal Hemorrhoids can't be felt on exam as they are too soft so no way of knowing other than having a scope.

This is making me miserable so any help would be greatly appreciated.
