Hi it does look irritated you might need a stronger cream and use it for a few weeks. I'd have it checked with your doctor. Just wash with water and maybe with a emollient wash.

You may need a prescription strength cream as some of the over-the-counter creams are quite mild. it would be best to see your doctor and have it checked out. the creams can take a long time to work so persevere with them. is the foreskin getting tight?
Erections or very normal during sleep to help reduce them make sure to pee before bed time and bring your knees forward into the fetal position this will help to reduce them. I'd not wear any underwear either when sleeping as it could cause friction.

when i say i get erections i mean for 45 mins - an hour. the redness seems to follow the line of my foreskin when my penis is erect. Whilst it doesnt feel tight i suppose its a little tighter when erect. Ill use cream for another two weeks before moving onto next step

You could bring this up with your doctor as well when you see them. it's normal to have several erections during your sleep lasting for half an hour or so. But when you wake up it doesn't go away? Even if you get up it takes a while to go down? In a sense your lucky as most problems are with not being able to get erect 😁

yeah i have to walk around for it to go away. And its as hard as it will go aswell, bordering on painful. Ive read a few other posts and it seems to be fairly common on here. I havent checked for STD as yet but have no pain, burning, discharge, smell or any of the usual symptoms for them
