are ct scans reliable for diagnosing appendicitis?

hi last tuesday i had intense pain in my upper left abdomen pain got worse and started to feel pain in my lower right side which made me run to hospital anyways the hospital did a ct scan without contrast said everything looked fine and bloods were okay but had blood in my urine? then next night pain got worse in lower right side so went to a different hospital for a second opinion they ran test said my white cell count was high and had ketones in urine no blood anyways next day they decided to do another ct scan this time with contrast to check my appendix everything came back looking okay to then so they said it could be a bug and go home. im now at home its been 7 day sense symptoms started.my feeling of wanting to vomit all the time and not eat has kinda passed but im still having the the pain! are these ct scans reliable for picking up appendicitis? i dont understand i had classic symtoms the high wbc and wanting to be sick but no fever and they send me home. do you think i sould look more into this or should just move on and take their advice that it could just be a bug.

I had my appendix out last Sunday, I had ultrasound and they looked fine no high wbc, blood, keytones and luecocytes in urine but pain kept coming and going for a week with awful nausea and heartburn, pain eventually got worse after 9 days so surgeon decided to do laproacopy, I had bleeding in my pelvis and loads of adhesions and they removed the appendix, said they were not grossly inflamed whatever that means but took them anyway, waiting in biopsy results to see was it the appendix that caused the problem. Go back to the hospital if it gets worse or more frequent, a ruptured appendix is a lot worse to deal with than removing before than, Drs should treat the patient based on symptoms and not a ct scan, they said I had grumbling appendixs which could be what you have, google it

Hi there, im thinking, blood and ketones in your urine could possibly signify a urine infection...I think so. Are you having any urgency or frequency of going for a wee? I mean, more than usual? My temperature doesn't show up, not always, so when I presented with appendicitis they didn't take much notice until they saw the blood test results. What were your CRP [c-reactive protein] results? That will show up how much infection is in the body. In the UK it used to be called [and maybe sometimes still is] ESR test. I personally don't think a CT is a definitive yes or no test for appendicitis. But the wanting to be sick could be a sign of a [need to get antibiotics and fairly urgent treatment for] a urine infection. If it is that, and it's just left, there could be damage to the kidneys. Do you know if the CT checked your kidney area? And also you may have [usually little] kidney stones that are trying to pass through. Hope this is not confusing you in any way. I wasn't vomiting/never vomited at all before whilst I had appendicitis; not everyone doe. But some doctors and nurses believe it can't be 'appendicitis' if you haven't vomited. Good luck with it all. Are you in the UK?

im starting to think its a uti. my bladder is feeling a bit off and side hurts near the kidney. i still got horrible stomach cramps but now im thinking its cause of a bad uti. the ct scan a few days ago said kidneys looked fine.

Did they do a urine culture, mine came back positive but they still took the appendix they wouldn't give me pain relief or antibiotics in the hospital incase the appendix ruptured and the medication would mask it

my doctor got a samply of my urine today and did the dip test and only mentioned ketones. dunno if those test show utis or not.

i really dont know what to believe or think. still think appendicitis but them uti rings bells aswell. im getting an ultrasound tomorrow so if that comes back fine im gonna rule out appendicitis because im sure 2 ct scans and an ultrasound would show something.

    Hi

It could just be "grumbling appendix" at the moment. I just had a CT from my consultant and he told me CT scans are very very detailed nowadays and can detect the slightest shadow etc.

Obviously I am not a medical professional or giving you advice merely stating my experience. Mine burst and I had a fever, vomiting, constant pain to the point where I couldn't walk, that was over a period of 24hrs, it happened very quickly!

I think if you've had 2 CTs and they have both come back normal! What about a urine infection? Have you had antibiotics?

well im in the hospital atm because i got horrid oain again... they just checked for uti and said i didnt have one they are not looking into my appendix because of my ct scan the other day being normal. they are doing bloods atm and did an ultrasound of my kidneys. i have no idea what is going on even the head surgeon is confused.

They said my u/s showed fluid so probably a burst cyst but when they went in it was blood and not fluid

just got sent home from hospital. no uti normal blood work and normal kidney and bladder no blood in urine just ketones prob from not eating much. i give up. guess i have to look at other things. surgeon said shes positive its not my appendix because ive had this for over a week and she made me do some of the tests lile jumping up and down and so on and they didn't make me worse np rebound pain and of course 2 normal cts. guess im gonna have to look at other options. maybe ibs or something? just doesn't explain why i had a high wbc and blood in my urine when this all started.... strange.

Hmm, this is strange. How's the pain now? I think you must be in America, cos you used the term blood work. I was just curious to know if you were in the UK because I was 'neglected' and tho I was promised many times, 'you're next for theatre!' this didn't happen until 24hours [they reckon 'just under] later. And I've had over 5 years of chronic pain. Last week I got diagnosed with fibromyalgia, which can be triggered by infection. By the time I got to theatre, my appendix had perforated and was gangrenous. The last thing I want to do is scare you, but I'm not the only person to have been neglected/let down by the NHS. And if the pain has [?suddenly] stopped, this is possibly when your appendix may have already burst. Are you lethargic, or kind of dragging yourself around, with a kind of mild disorientation? Have they considered tests like barium meal/barium 'swallow'/follow-through? Hang in there, we are rooting for you wink

still got pain in my abdomen and on my sides. kinda feels like ive been bashed around a bit and im not in america im in australia. i dont really have anymore options right now ive been 5 times to the hospital and they always say they cant find anything they are gettting as frustrated as me at this point. im going to get an ultrasound on my appendix tomorrow just to look at it with a different screening as for blood test i dont really know what the standard bloods they do in hospital but they said mine are fine. maybe you could give me a lost of what to ask my gp to check on friday when i see him that would be great.

its okay not intense like it was. i just feel unwell and got an uoset tummy atm. i dunno if it really is apendix or ive just got a real bad bug thats lasting a week and a half. just makes no sense with having a high wbc at one point at blood in pee but thats gone now. from what i hear about apendix pain it doesn't go away or calm down just gets worse and worse

Remember I mentioned ESR and CRP on the blood tests? Did you ever find out from the GP or the hospital what those results were?
    • Posted

That's good. Will you be able to take someone with you? Sometimes it's hard for ourselves to speak up for ourselves. Thinking of you tomorrow and hoping you get to the bottom of all this.

the doctor didn't wanna do the blood test because i had a white cell count of 10.7 so he thinks that makes me fine lol what a joke anyways i was very firm and demaded a full more in depth blood check up he finally agreed to do it. getting the esr test dunno if he put that other one in. hopefully get results in 3 days and hopefully wont end up in hospital again before then. so frustrated because i have no idea what is causing all this.

got my blood results everything came back fine except in my iron studies my ferritin is 314 when its meant to be 20-250 doctor was like do you eat alot of meat i said no. he said just to get retested in 3 weeks im like ummmm okay. i have no idea what it even means.

Too much iron is most often caused by an inherited disease called hemochromatosis. Many people with this disease never have any symptoms, especially women who lose iron through menstruation. But men and some women slowly build up excess iron over the years. They may begin to feel joint and abdominal pain in their 20s or 30s. Heavy alcohol use increases the amount of iron that is absorbed.

I was wondering about that condition too, Faye. In the UK the treatment would be to remove blood, almost like 'giving blood' as the only way to get the iron out of one's body. Matie if this is what you've got, it needs your attention, as it can seriously affect your heart...Don't want to distress you, just so you know, in case that's what it is...Okay, I'll just say it! My brother had a massive high iron levels, drank heavily for years and suffered a sudden fatal heart attack in December. He refused to get a repeat blood test. He was only 45. This is why I was also wondering about this condition. How old are you, Matie?

im only 28. i was a heavy drinker but quit 3 weeks ago so sober now. my level is high but dunno if its serious high anyways im gonna keep an eye on it ill get retested in about 2 weeks do you know much about ferritin levels ? how do i lower it.

nevermind just read mine could be high because i have a fatty liver. i guess my main concern is still if i have appendicitis my abdomen pain seems to have calmed down and nearly stopped for 3 days and im eating normal again. also my wbc has lowered down alot. would you say i can safely rule that out now ?

heya just wanted to say thanks for all the info. i still have the pain on my right side and pretty horrid symptoms but seeing a gastro doc atm who think not appendix because its been going on for over a month. i did have elevated wbc at one point but thats dropped and my cpr is 2 so theu are saying it normal. they also ended up doing another ct but couldnt find the appendix which they said is a good sign because if something sinister was going on it would show. i am now trying to stop thinking about it and its hard because the pain is pretty bad and so on but surgeons and doctors are not to worried sp gotta listen to them i guess.

Here's my story: I was at a sporting event (watching), when all of the sudden, I felt like 5 people were stabbing me, I couldn't sit, stand or lay to find any comfort. I called my mom to come pick me up (I am 15 and do not have my license yet). I got into the car and started screaming. I didn't know what was happening, but there was a sharp pain in the center of my abdomen. My mom wanted to take me to the ER right then, but I told her I wanted to wait and see if the pain went away. It didn't leave, but it did move it's way over to the right side of my abdomen, and I kept feeling extremely nauseous. A day later at 4 in the morning, I finally let my mom take me to the ER. The first ER doctor told me I had "textbook appendicitis" and all we would need was a CT scan and the surgeons would take it out as soon as possible. They gave me a CT and they saw a small appenicolith and enlarged lymph nodes, but... they decided to wait until the next morning to make the ultimate decision and run a few tests to see if things got worse. So, they admitted me into the hospital, and I stayed the night. The next morning, they didn't see anything strange in my labs, so they really had no reason to take it out. No fever, no weird blood cell counts, and they honestly didn't know what to do with me. So, again, they had me spend the night. The next morning, I was feeling a little better, and they told me that it could be a number of things; ovarian cyst, mesenteric adnitis, and a few other things. They said that if it was mesenteric adnitis, that it would go away in less than 2 weeks. That day they sent me home. The day after that, I had the same sharp pain in my lower right abdomen, and since then, it hasn't gone away. I would go to appointments with my pediatrician and she would ask if things had gotten better, and each time, my answer was no. Every morning, I wake up sick and lean over the toilet and gag for around 15 minutes, and I'm always in pain, but some days it's a lot worse than others, and around 4 times a week, I will have a massive amount of pain for a few hours straight, then it dies down. A few weeks ago I had my first appointment with a GI specialist where he told me that he wanted another CT scan, and brought up a few things he thought it could be (mesenteric adnitis being one of them). I then was confused, saying that the doctors in the ER had told us mesenteric adnitis would last 2 weeks at most, and it has been A LOT longer than 2 weeks. He then told me he has seen it last up to a year and a half, and that it was a possibility that I had it. A day or so after that appointment, I was having really bad extreme pain, and I thought it would die down in an hour or so, but it didn't. It had been 2 days of the extreme pain with no relief, when I told my mom I needed to go to the ER. I went. They preformed another CT scan, and this time found nothing wrong with the appendix, but still enlarged lymph nodes. It's been a few weeks since the ER visit, and since then I have taken a serious turn for the worse. I also had a colonoscopy (that I should be getting the results back any day from). My doctor who did it, said that he didn't think he saw anything, but he sent it in to the labs to get it looked at. If nothing is found in the scope, they told me I would be diagnosed with mesenteric adnitis. I don't know what to do, I can barley function at school and haven't been able to go to a full day in 2 weeks.

This was a few months ago, and I ended up being diagnosed with mesenteric adnitis (after 3 and a half months of dealing with this) My mother, however, did not listen to the diagnosis and went to a sergon and had my appendix taken out. They tested it, and it came back positive for appendisidis. They re checked my second CT and saw that the radiologists had missed the two stones inside of my appendix. There are many cases of chronic appendisidis where there is nothing seen on the CT scans. I never ran a fever, threw up (though i was nausous).

I would think about getting it removed, if possible smile

Best of luck. None of this stuff is fun sad
