Article on PanOptix. Might be of interest if thinking about that IOL

Came across this article on the PanOptix compared to the Symfony published last month.

It talks about the importance of controlling astigmatism to work well and how personality traits are a critical feature in determining if a patient will be happy with the IOL. These rules apply to any defractive IOL IMHO.

“The lens creates permanent haloes and starbursts around all point sources of light; these are especially evident at night, but may also cause a slight softening of vision during the daytime. Patients who require crisp vision above all else, or patients who need sharp nighttime vision (e.g., bus drivers or truckers), will be happier with single-focus IOLs.”

    The truth is that you can get halos / dyspohotopsia from any lens. There is no rule. It may happen or not. You can only control probability by implanting a monofocal IOL instead of EDOF. But no guarantees at all.

    That is a pretty good article and I have bookmarked it. Too bad that they did not compare the PanOptix to the Synergy. That would have been a better comparison. A few things I took note of:

    • When they say astigmatism can be a problem, I think they are talking about a fairly large amount, more than 1.0 D.
    • Sometimes I think a bit too much is made about bifocals and trifocals only having certain distances that the provide good vision at. If you look at the defocus curve, one "optimized" point for the PanOptix is at -1.5 D. It is suggested that it is not going to provide good vision at further out (gun sight example). However, at -1.0 VA only drops off from .06 to .09. That is pretty insignificant, and well above the nominally acceptable 0.2 level. It is essentially the same as the Symfony at that distance. A similar observation could be made about the distance between the two optimum points. It dips but not below the second optimum.
    • Not sure where those defocus curves came from, but the AcrySof monofocal seems a bit on the low side compared to other curves I have seen for that lens, that show the 0.2 VA point at -1.0 D. A pessimistic curve for the monofocal of course makes the MF lenses look better...

      Me too as that is the IOL I am most interested in, as long as the dyphotopsias in patients is no worse than my current Tecnis MF.

      Synergy should be better in dim light than my MF and having better close up vision would be nice.

      I can see close up now, but to read a book or magazine comfortable, I have to find that right sweet spot and have good lighting. This IOL claims to get 20/25 down to 12" or so.
