Asked Dr for a letter for sons pip review and Dr wants to know what to write

Hi, asked doctors surgery about getting a letter as evidence for new pip review form, they said they will want to know what I want in it, can anyone help, I don't know what they have on record so should I go off his previous award and copy the descriptors they awarded him on his original claim, also they are saying it will be a couple weeks before I can get the letter and I need post it by this Wed, can I send it after I've sent the form back or take it to a f2f if they send him for one, so stressed I can hardly think,can anyone help me with what to put on a list when I ask him for the letter.     Hi Angela, maybe you can ask them for his medical records since he became disabled, also this will include any report's from his external medical professionals thus giving information about his diagnosis, you can also add a letter from you and anyone who helps with his care.

    If you could possibly write out a letter for your doctor to use as a structure for their report as well, this needs to explain how his disabilities will effect him on a daily basis and maybe they can get it done quicker so it can accompany his PIP forms

    Hi,  I had a similar problem with when i was claiming on behalf of my grandson,  I phoned his specialist and they wrote a letter which I took it to his f2f and they accepted it.

    Hi,

    No one can help you to know what to put in the letter because we don't know how your sons condition affects him. The letter should state how he's affected by his condition, does he need help to be able to go out? what support does he receive daily? that sort of thing. You can also write a supporting letter to go with it.

    It's not ideal sending the evidence after the form has been returned because his file will be sent to the assessment providers once it reaches DWP. DWP have nothing to do with it at that points. It then becomes the assessment providers job to complete the assessment and the report. You can certainly try to hand it in at the assessment but don't be surprised if they refuse to accept it. It's happened to me and lots of others too. This is why i always advice to send it with the form.

    Does his GP have any letters from past appointments on your sons medical records? You can ring and ask if there's anything you can use but there maybe a change for this, it's anything up to £50.
