aspiration over surgery?

hi, I've noticed that many posts on here include individuals who have had surgery over at least trying aspiration and sclerotherapy. Could someone help me understand the reason? The latter is way less invasive (and seems to have less complications) and even if it fails, surgery is an option. Could it be that not many urologists offer aspiration and sclerotherapy?
