At a loss...

Around early March this year I went to the toilet and midway through urinating I felt a sharp pain followed by some blood in the urine. Since then I've kept having severe discomfort/burning sensation in my penis after I urinate and sometimes when I become aroused. It feels like something is stuck in my urethra.

Sometimes when I urinate I have a sharp pain at the tip of my urethra but often it feels fine; I also feel fine when I masturbate or have sex until I ejaculate when I get the same sharp pain at the tip.

I've had many, MANY, urine tests since, had a prostate exam, blood tests, an ultrasound on my kidneys, bladder and prostate, STI tests, a physical examination of the penis and so far nothing has been found. I've been on two antibiotics which don't seem to have helped. This seems to rule out a UTI, an STI and prostatitis which were the most likely candidates. 

I have an appointement with a urologist booked in for later this month but in the meantime I'm going a bit spare; I'm starting to worry this could be urethral cancer or something like that. 

Anyone got any ideas or had a similar experience?

    You do not state whether you have had an Ultrasound or CT scan, so have you considered the possibility of the problem being associated with kidney stones or maybe just sediment flowing down the urethra with the urine?

      I've had an ultrasound on my kidneys, bladder and prostate which didn't show anything (at least not to the doctor doing my scan at the time, still waiting for the results to get back to my GP). I haven't had a CT scan. 

      I did think that given there was such a sudden pain right when this started. I'm not so sure now because I would have thought any damage/irritation would have gone by now and my ultrasound didn't show any signs of stones. 

      I've never really heard about sediment before, would that show up on any of the tests I've mentioned?

      Correction, you do state that you have had an Ultrasound, but maybe that would not pick up sediment in the urine.

      Do you live in a hard or soft water locality?

      Sediment is difficult to pick-up on tests, but when it does pass it can cause minor (although painful) slight tears in the soft tissue.

      Soft water. That's an interesting idea, I'll make sure to mention it to my GP and urologist. What could I do about it if that is the issue?

      Also, I suspect I'll need a cytoscopy, would that pic up small tears?

      Small tears do heal of their own accord but can be painful while they do so.

      I do know this only too well from personal experience.

      Yes, a cytoscopy could well pick-up small fissures.

      Usually the advice in dealing with cases of suspected sediment is to drink plenty of water in order to flush-out whatever might be causing the problem.

      This pseudo-diagnosis is certainly not set in stone, but it can do no harm to increase the water throughput until you visit the Urologist again.

      Thanks, at this point I'm really just looking to reassure myself that it's not necessarily cancer lol. 

      How long would tears take to heal? It's already been two months. And should I refrain from sex/masturbation?

      I really do think you need to put any idea of a maligancy completely out of your mind.

      As for sexual activity, I feel there is really no reason at all why you need to desist, other than if you feel the pain is becoming an issue.

      Please remember what I said about this being not set in stone, as only your own medical people can give you an accurate diagnosis.

      If I am not correct in what I say, then these are other possible causes of your problem:

      UTI causing Dysuria

      Ureathral Obstruction

      Urethritis

      STI

      Dyspareuia

      Diabetes

      Having said this many of the above have already been eliminated so really should not figure in your case. That's why I feel it could be more associated with sediment.

      If is a tear, as I said it can take quite a while for this to clear-up, especially if more sediment is still being emitted.

      In my case I suffered with this problem for the best part of 4 months, and I still have the occasional repeat performances.

      When are you due to go back to see your Urologist?

      Thanks, like I said I'm just really glad to know there are other possibilities! You've been a great help in that. 

      I'm due to see the urologist on the 19th, I really do think I'll need a cytoscopy so I imagine it'll be a little while after that that I finally know what's going on!

      With regard to sexual activity I'm just concerned that I may be agravating any injury that is there. Or do you think that's overworrying?

      I really do believe that we men to tend to 'over worry' about certain scenarios, and in this particular case you appear to be falling very much in line with that.

      Sexual activity is important, so unless there is the possibility of an STI, then  I would say just carry-on as normal.

      Reading back I think I saw somewhere that you have already been through an STI checkup, which I presume included Herpes, then you are very unlikely to make anything any worse, other than the occasional pain.

      If you are ok with that, then there is no problem - just carry on flying the flag and above all relax and stop worrying.

      It is the worry that can escalate and cause us the biggest problems of the lot.

      You are doing the typical 'Bloke' thing of over-worrying - we all do it.

      Anxiety-related (Mental Health) issues are by far the greatest dangers in the UK today, so don't fall into that trap.

      Yeah I'm def the anxious type when it comes to illness, sometimes I worry I'm a hypochondriac! wink
