Atypical odontalgia

Hi there, I had two front teeth veneers done back in Jan 2017.  I have Sjogrens Syndrome which means I produce very little saliva - over many years this has caused my front teeth to erode and chip easily.  The dentist recommended I had veneers done. How I regret this now.  Since then I have had constant pain in my front teeth, & a horrible sour taste in my mouth. I have been back to my dentist on about 20 occasions, had x-rays and every dental test available, my mouth is healthy, the veneers are perfect and I have no decay - but still the pain.  I had been coping by taking paracetamol and ibruprofen but about 2-3 weeks ago the  pain became intolerable, especially at night. I was getting no sleep and painkillers were not working.  I went back to my dentist and saw a different emergency dentist who suggested I had root canal treatment in one of the front teeth the pain was coming from.  She couldn't promise this would work, but I was so desperate I was prepared to give it a go.  Well the root canal treatment didn't work and pain was as bad as ever.  I decided to then visit my doctor.  She thinks it is nerve pain and has given my Codeine 30 -60mg every 4 hours and started me on Amytryptiline 10mg at night, increasing to 20mg after a week if no improvement.  I haven't been officially diagnosed with atypical odontalgia but from everything I have read it fits my symptoms perfectly.  I want to know if anyone else has had good results with Amytriptyline.  I am so scared I am going to be in pain like this for ever.  The pain killers, even codeine don't seem to work.  Any tips or advice will be gratefully received.

    Hello gill,

In case of nerve pain regular painkillers usually don't work. I have a nerve damage and for me Gabapentin and Nortriptyline have helped. I also took Amytriptyline, but I felt that for me Nortriptyline is better as Amytriptyline made me very sleepy. 10 mg is a very low dose. I took 50 mg, plus maximum dose 3600 mg Gabapentin. So don't stress when 10 or 20 mg don't work.

Before I found out that I have something wrong with the nerve I also went for 4 months to different dentists- many root canals, lost a tooth etc. I ate SO MANY PAINKILLERS and no help at all! Later a doctor told me that painkillers don't work with nerve pain.

.Amitryptaline by itself is unlikely to halt the pain. It's used at night usually, because ir makes people very sleepy. The pain killers you need are nerve blockers like Pregabalin ans Carbemazepine. You need to see a consultant in nerve pain, who can arrange an MRI scan and tell you what's going on.BTW,for dry mouth, one of the best things is Biotene toothpaste.Bit more expensive than the usual.but a good dry mouth paste.(it has enzymes in it which encourage production of saliva).
