back/shoulder muscle problem.

I have had something playing up in my mid back/shoulder region either underneath my shoulder blade or somewhere very close to it. It affects a lot of the muscles in my right side and i have come quite close to pinpointing what it is but i'm still not sure and i was wondering if you guys could help me to diagnose and fix it permanently..

I'm pretty sure it has all stemmed from either manual labor (carrying heavy objects on my shoulder) or a skateboarding injury.

I don't really suffer from pain too much, it is more of an uncomfortable feeling. i practice yoga and i can feel an uncomfortable slight strain in a lot of poses and i just generally know its there. The most prominent of noticeable symptoms is that when i drop my head forward, i can feel a pull stemming from my neck to my lower rhomboid muscle i think. Also, sometimes when i workout, i get a stitch/stabbing sensation in the lower rhomboid when i am inhaling.

I have recently been using a hard massage ball up against the wall holding it in all the painful spots for 30-60 seconds. There is one specific spot in the lower rhomboid(i think), and if i am hitting that spot and then drop my head down, the pulling sensation isn't there anymore. So i am convinced that i have found the location of the problem. The massage ball helps greatly, but doesn't eliminate it. This makes me feel like it is more than just a knot in my muscle ? 

What are your thoughts ?
