Banding, anal ulcer, and now fissure! Botox advise please

I have suffered for about 2 months now of unbearable pain. It started with a little blood in my bowel movement and I thought to myself, I am about to get an onset of hemorrhoids little did I know, it was going to get considerably worse. The pain gradually increased with bowel movements and then as the days went on I couldn’t even sit down. I found night times awful, the pain wakes me up and then I can’t get back to sleep so end up having a bath to try and relax. This got worse and worse by the day, but with a little bit of positive news I also found out I was pregnant.

Over the course of 2 weeks I went to the doctors 3 times and the out of hours service, I had a carrier bag of medication from stool softeners to suppositories to gtn spray to hemorrhoid cream but nothing was touching the sides. The pain was obviously worse after a bowel movement but it got to the stage where I couldn’t pass wind without being in agony. It got to 2.5 weeks in and I couldn’t walk. I have never experienced pain like it, to the point where my partner cried because he was so upset that I was in so much distress. I called an ambulance as I just didn’t know what else to do. The paramedics were great and actually said that I had done the right thing after trying every other option.

Into hospital I went, all whilst only on paracetamol as I was pregnant. I eventually saw a surgeon about 6 hours later who recommended examination under anaesthetic but advised that this may not be safe for the pregnancy. At this point I was in so much pain and so early in my pregnancy that I had to take the risk. I went in the following day for surgery and they banded 3 internal hemorrhoids and gave local anaesthetic to deal with an ulcer. Felt terrible after the banding as it just gives you uncomfort and was still in pain. A week went by and I was no better so I went back in and they said that there was nothing else they could do and gave me some hydrocaine. This is an anaesthetic and I cannot recommend it enough, it is the only thing providing me with a glimpse of a normal life.

It is now 4am and I am awake as the ulcer appears to have turned into a tear- the pain is how everyone describes an anal fissure. I am currently under the care of a private consultant as I felt slightly fobbed off by the NHS. He has suggested Botox next. Is this done under local or general? Has anyone got any positive outcomes? I’m 32 and sick of this ruining my life, and just to put the icing on the cake I’ve also miscarried. Would like to get as much sorted before I get caught pregnant again?
