Best Q fever doctors and treatment

Hi all. I am writing you because of a severe Q fever that was diagnosed to my mother. After a long period with a flue-like symptoms she was hospitalized because of pulmonary and pericardial effusions. Although the doctors still did not know the reason for it they have given her antibiotics which for 14 days in the hospital. She first had slight improve but she got worst again. She was hospitalized in other hospital where she was once again on antibiotics and corticosteroides. Many many blood tests, analisys, etc. were made to her without showing what is the reason for the effusions. Finally, after 2 monts treatment with the wrong antibiotics and corticosteroids she was diagnosed with Q fever. She was prescribed doxycyclin which she took as tablets for 13 days. After that she was hospitalized again in infectious hospital in order to be treated with doxycyclin intravenous. She is at the end of the treatment now and the beginning of next week she will be out of the hospital but 5 days ago she began to have effusions again and her temperature went high again. I don't think it is normal to worsen while she is already 20 days on doxycycline. We are desperate for the lack of improvement for months. I don't think the doctors in Bulgaria are familiar with this disease and the treatment is not good enough. We are considering treatment in other country where they are better professional and more experienced with that. Does anybody know where this could happen. As I mentioned I am from Bulgaria and it would be good to be some where nearby but other proposals are also welcomed.
