Big 1 Year PSA Spike

Hi everyone,
Im in a huge state of panic right now. Im late 30s and last year my doc started checking PSA due to family prostate issues. it went from less than 1 last year to almost 14 this month. I just recently got the results and my clinic is still closed another day or two before doc even sees the numbers. Thats a major jump so my mind is jumping to aggressive cancer.
Has anyone had a jump like that that wasnt cancer? I have had some issues with hard bowel movements since the last test and had to strain to go just a day or two before the test which i think can affect prostate? I had not had an ejaculation for pretty much exactly 72 hours before test.
I had a colonoscopy due to mentioned issue back in early summer and then the doc wrote he didnt feel a prostate mass. Is there hope this isnt cancer? I read one medical paper that tested men under 40 for high PSAs and only like 1 out or 75+ had cancer but my number is slightly higher than their highest patient...

i understand your concern.
i just wanted to let you know that i had a similar concern a year ago but my situation is different from yours.
First off i am 59 years old...the older you get the higher the PSA value will be..i don't know how much the age contributes to the PSA in terms of value.
Last year my PSA went from 6 to 13.7 and like you i was concerned. Turned out to NOT be cancer after a biopsy.
I will be brutally honest with you....at your age that shouldn't have happened and is concerning..the acceleration of PSA is definitely something you should have checked...but don't jump to premature conclusions. Hope all will be well and you don't have cancer. Maybe prostatitis could cause it..idk..i'm not a doctor...Good luck.

yes, i had this in 2021, PSA went up from 2.9 to 12.0. Had an MRI which was red-flagged for PIRADs 4, probable prostate cancer. had a template prostate biopsy which came back negative for cancer, just benign growth. Last PSA done this month was 2.9 again.

AFAIK Sex will spike your PSA numbers as will prostatitis. A few other activities will also.

"Had not had an ejaculation for pretty much exactly 72 hours before test" is not sufficient for the PSA accuracy. Make sure no prostatitis/UTI symptoms and sexual activities for two weeks before recheck the PSA as well as ask your doctor performs a digital rectal exam.
