black circles/ flecks in stool

Ive had these black circles/ flecks in my stool for about 2 months now. I have had parasite tests, blood tests and an ultrasound and they have all come back negative. Its worse when I have loose stools and its more noticeable. There is also grain looking specks in the stool too and sometimes there are skin looking things on my toilet paper after i wipe but those are more noticeable after i eat things like tomato's. Please help! Its freaking me out.
