Black spot on glans. Is it melanoma? Have 4 of these.

Hi,
I am a 24-year-old man. I recently discovered 3 black spots on the glans of my penis. I am sure there are 3 spots. There is a 4th one on the foreskin on inner side.
So, I have multiple problems apart from the spots.
First, I am uncut. Also, my foreskin is a bit tight. It has always been tight, so I never pulled it back often. This has led to another problem of smegma accumulation.
Also, tight foreskin has led to very sensitive glans. On top of these problems, my flaccid penis size is barely 2 inches. This makes it extremely hard for me to pull back the foreskin completely.
So, here’s a list:
- Black spots on glans.
- Tight foreskin.
- Smegma accumulation.
- Sensitive glans.
- Flaccid size is 2 inch.
Out of all these, my main concern is problem 1, those 3 black spots. After reading on the internet, I suspect I have either of the any cases:
A. Penile Melanosis.
B. Penile Melanoma.
C. Atypical Nevus
D. Lentigo
E. Lentigo Maligna
F. Angiokeratoma
G. Hyperpigmentation
H. Mole
If its case A then its nothing to worry about, I guess. When I checked for the symptoms for case B, I had none of the symptoms like swelling, bleeding etc.
However, what makes me a bit skeptical is that I read somewhere, having smegma built up overtime can cause irritation and can increase the risk of cancer (case B).
Also, regarding the spots, they are black in colour, flat round shaped. No pain. No bleeding. No swelling. Out of the 4 confirmed spots, one was present from 3-4 years. The 3 new ones might have appeared recently.
Reasons which make me feel its benign:
--> Penile cancers itself are rare.
--> Most of the cases are observed above age of 55. I am 24.
--> 95% of the cases are squamous cell carcinomas.
--> Of the remaining 5%, the next likely cases are: small cell, Merkel cell, clear cell, sebaceous cell or basal cell
--> Extremely rare cases are: melanomas and sarcomas
--> Penile melanomas are rare. The most commonly affected areas are the back in men.
--> Mostly caused due to overexposure to sun/UV light. I am indoors most of the time. Also I am not Caucasian, if that's of any help.
--> Although due to tight foreskin and thus lack of cleaning I have smegma. But, no evidence of
smegma causing it.
--> Does not pass ABCDE test.
--> No bleeding,pain,swelling,discharge,lump,itching.

Are you able to refer yourself to Medical Professionals such as a Urologist or Dermatologist?
If not, see your Regular / Family Doctor GP when you can ....

I have consulted a derm online. They said its nothing to worry about. But I am still unsure and confused.

I would suggest that when you are able to book an in-person appointment with a Urologist for you to check out ALL your issues of concern - however, I would agree with the explanation given to you by the dermatologist online with the proviso that they were able to view the images you have posted here.

Here is my more considered response to what you say: in your initial Posting:

1.Black spots on glans. - I would suggest there is a relatively simple explanation for these.
2.Tight foreskin. - This is something you can work on by doing stretch exercises, but yes, I fully understand this may not be particularly easy due to your flaccid state size. In brief, you have your whole lifetime ahead of you the sooner your start to work on this, and all the issues you have with your penis, with the help of an understanding Urologist, for example, the happier you are going to be rather than carry these issues with you throughout your adult life.
3.Smegma accumulation. - 2, 3 & 4 are linked, the sooner you are comfortably able to expose the entirety of your glans the sooner you will be able to establish an appropriate penile hygiene regime.
4.Sensitive glans.
5.5.Flaccid size is 2 inch. - This is perhaps the most unfortunate issue as the majority of these issues are linked to natural growth hormones and it is most likely it is too late for you to address this. Again, a good Urologist would fully inform you what the options are on this within the bounds of accepted medical practices.
However, there are a whole range of ways and means to increase penis size, and therefore, in-turn impact on your level of contentment with your penis. Again, the essential issue is once you have the low-down on the medical approaches this will determine if and when you look at alternative means, and, again, the sooner you potentially start to explore these approaches in practical terms the more likely you will implement change that you are happy with.
Essentially, as I explain to anyone contemplating embarking in penile enhancement techniques is that it it is all-too-easy to create additional issues, some of these can be overcome by allowing the penis to recover naturally, while others become permanent and therefore become a psychological burden for the remainder of your life.
Simple approaches such as learning to massage your penis with a light application of something like Coconut Oil can be the basis of whether or not you are happy to work with your penis or not, and if not, I would suggest you do not try ANY other techniques at all ......
