Bladder Neck Incision Help

I had a bladder neck incision (TUIP) 2 weeks ago to help with slow flow, chronic prostatitis symptoms, incomplete bladder emptying etc. I have a small prostate but bladder neck was very tight. The first two days in the foley were torture mainly because I could never get used to it. After taking it out my flow was greatly improved initially and I felt like I was emptying my bladder well finally. After several days it seemed like I started not being able to empty my bladder fully anymore. Now for the past several days every time after I void I have to head straight back to the bathroom 5 minutes later to go again. The weirdest part is that it doesn't feel like the urine is left over in my bladder it literally feels as if it is getting stuck somewhere in my urethra. Maybe between the internal and external sphincter? Anyhow, it is very uncomfortable and causes a lot of irritation. I have a small amount of blood in my urine but that's normal at this point so of little concern. I believe I have been passing small blood clots as well. I called the surgeons office about this and he is supposed to be calling me back tomorrow. I just wanted to reach out to see if anyone else has had this with this op. Could it be some form of incontinence or something? I know the recovery time can take weeks even months sometimes but I could really use some encouragement. Oh I forgot to mention I didn't get the dreaded retro so I'm definitely glad for that at least. Thanks!

    I think that it is mostly likey that your bladder, bladder neck and nerves are very angry and are still healing. youve just had a part of your body cut open and ive read that a bladder neck incision can take up to 3 months to fully repair. is your flow still good?

      Yes flow is still pretty good its just like my body doesn't know when to cut off urinating or something then urine feels like it gets stuck and starts irritating everything. I guess its just alarming because I felt better right away then down hill. I guess the recovery will not be linear unfortunately.

    oh, and was was your post void amount before your operation?

      When they measured it was like around 250 but I had a very full bladder I don't think its usually that high. I was just fed up with always having to go within 30 minutes of going first to get the rest out. A lot of getting up at night to go as well. I'm sure there is some bladder dysfunction too but bladder meds didn't help me. So I'm hopeful over time this will improve.

    That is quite a high PVR. I'm going to hazard a guess that your flow was very slow before the operation. Did they ever tell you what your Qmax was? your surgeon should give you another scan to see how well your bladder is now emptying. I have a mild obstruction caused by my bladder neck but I'm treating it with flowmax. Were you voiding small amounts before the operation? something to think about is your bladder will have gotten used to you voiding only small amounts so youll probably need to retrain it, but before you do that you need to know if your PVR is under 60mls. I have spoken to a few guys who have had the operation and it takes a few months for things to settle. maybe give an alpha blocker a try in the mean time as it helps settle my symptoms down.

      As far as remembering what my Qmax was I cannot. I do know now it is much higher especially when my bladder is full it is at least twice as strong I would guess as before. Yes I think your right the obstruction has caused my bladder to learn some bad habits and alert me to empty far too early. I'm hoping this will improve with time as everything adjusts. Funny enough today my symptoms have actually died down a lot. My doc said everything I have been experiencing was normal and it can take 3 months to recover. It may just be a roller coaster.

    I think UROs are dreaming telling us we will be fine after 4 weeks. For everything to heal 6-7 weeks. I was fine until 2.5 weeks then urge and frequency increased daily until I called dr and he gave me some meds for OAB. All I can say is I am getting better but its slow and some days seem to go backwards. for transient incontinence I think anywhere from 6-12 weeks.

    Im in week 7.5 now and I still dont feel right. it's a rollercoaster. slight mild burning at the end of voiding about half the time. improvements for me are gradual, like 5% better each week.

      is that for your bladder neck or your prostate? id be interested to know what causes burning at the end if urination because it seems common for a few voiding disorders

      hard to know, I had holmium laser bladder neck incision and urolift.

      I read someone on here said it took 10 weeks for the mild burning to stop. that part doesn't worry me its the urgency depressing me as its so unpredictable.

      Yeah I agree it's probably gonna be more like 4 months not weeks. They always low ball recovery times though. Funny enough my symptoms have been much improved the last 24 hours. Like you said it's just a roller coaster and I don't know what to expect one day to the next. Hopefully 6 months out things will be stable and we will be happy with the op. I also have the mild burning at the very end of urination too. Weird. I'm sure all of this will settle just gonna be a while unfortunately! Thanks for the info and keep me updated!
