Bladder Neck not fully opening and Pain

Life changed overnight 4 months ago

my story

47yrd Male

4 month ago all of a sudden constant peeing, dribble after peeing and Penis pain

No UTI (Anitbitocs completed)

Urodynamics and Cystocopy completed

No Stricture

Enlarged Prostate but not affecting urethra

Qmax of about 9 mL/sec. It was quite slow, long and protracted.

Have been on Tamasolin and now Mirabegron

Diagnosed as Primary Bladder Neck Obstruction - PBNO (ie bladder neck not fully opening during urination), and I will consider BNI surgery as drugs are not helping.

The pain/dull pain in penis is constant and not bearable, the frequent visits to toilet and during day and night i can prob live with, but its the penis dull pain i have trouble with

I also have night time erections (non sexual) and i think its because im stopping the urge to go toilet as im half asleep, maybe this is why my penis is in pain?, who knows with this stuff

    U have my symptoms.I really consider a bni , i am depressed and tired , all the time i feel the need to urinate ( felt in penis ) , freqvency ,penis pain especialy at the tip, and also sometimes pain after urination.also have stinging and burning in penis alot of time.life is a hell.i share your pain !

    Hi Frank - no testicle pain, more penis shaft , dull pain sensation almost all day

      Ok. I have very similar symptoms but also testicle pain (mainly on one side). Pain in the shaft also especially on the tip of the penis (like Valentin). It's worst at night but I do have some days where the pain is much less during the day but it comes back each time I feel the need to urinate (which is around 20 times per day). Pain in the penis is very bad if I can't urinate.

      I had a cystoscopy, bladder ultrasound and a prostate biopsy but in my case uro diagnosed me with prostatitis and told me no cure when antibiotics don't work. It seems that I also have an enlarged prostate.

      Same symptoms but different diagnostic.

      It also came from nowhere last July (6 months now) in my case. Did over a month of antibiotics without any results. Alfuzosin helps some with difficulty in urination but not much with the pain.

      I am currently trying pelvic floor therapy and stretches (started 2-3 weeks ago). It seems to help but some days it's like if I am back to square one (pain and urination all day) and I do not know why or what triggers it. It's been a real nightmare since that all started. I understand what you are going through.

      Frank from what i read our diseases , also give us alot of prostatitis symptoms , it affects indirectly.at your cystoscopy your bladder neck is normal ?

      Hi Valentin65377,

      Urologue did not mention anything about bladder neck after cystoscopy. Gave me a prostatitis diagnostic right away. I did not even know the bladder neck could be problematic and cause the same symptoms before reading this forum.

      so my guess is that it was normal.

    HI Frank - With your symtons it seems like they have just said Prostatis and thats it, but its worth still exploring other causes

    the combination of enlarged prostate vs Overactive bladder vs bladder neck is so complicated i think Uros just have to guess sometimes, unless its really obvious in scans (like a clear stircture or blockage)

    i think we all have similar symptoms, my Uro after the cystocopy (prob by ruling out enlarged prostate (which i have but not impacting), urethra clear, no stricture or blockage, antibiotics given earlier in year to no effect, so it kind of only leaves bladder neck obstruction (PBNO), the type with not actual bladder neck blockage but when it doesn't open fully

    so i don't think my Uro has proven my bladder neck don't work, but by all the tests and his experience he suspects it is that,

    I'm on Mirabegron for a few weeks, but if that don't help i will opt for BNI (bladder neck incision)

    i'm 47, don't want more children, just fed up of these symptoms so just going to try BNI

    I just cant belive how it came on overnight in Aug'21,

    i hoping (praying) BNI works, i suspect my 2022 will be very different. i try to look on positive side but is difficult with this condition some days, i suspect i will get use to it

    Hi - have anyone considered/researched Botox injections into the bladder, or will that make things worse, or not work, Reading about this treatment its more about helping people with OAB which by definition we have symptoms off BUT,

    One of the risk is Urine retention (which is exactly what i think i'm suffering from as bladder neck not fully opening i'm told) so maybe Botox is not appropriate

    Once again i get confused with Enlarged Prostate vs OAB vs Bladder neck issues

    Is not botox in the bladder,is in the neck to relax it.it s something different

      Thanks Valentin - almost feels its better to try botox first, and then BNI if botox doesnt help. Too much confusion and choices, but good on here to learn from others. Ive got my URO appt in 3 weeks so hopefully he helps me decide what to do, sometimes i feel its all guess work

    bLADDER NECK OBSTRUCTION can be a sign of 'ball-valve' effect. As that relates to the stream voiding. Stop and start, slow stream, sometimes OK, other times up every two to three hours at night. The bladder neck extents into the bladder causing all the havoc. I have 38 cc prostate, yet with obstruction I do not escape the BPH and OAB sypmtoms. Also, the Ultrasound renal was done to see image and size. Also taking 4mg tam and Myrbetriq

      last night i woke up every hour for max 100 ml every time.i agree the bladder neck obstruction gives u prostatitis symptoms ,oab symptoms etc....is a f*****g hell

    Yea, add to that Interstitial cystitis and it's enough to become crazy. I wonder if doctors themselves really have any clue about what the exact problem is sometimes.

    Hi guys - hope your ok and coping with symptoms

    sorry if we have discussed already but how do we know if we have Non Bacterial Prostatis, is there a actual test for that?

    My Prostate is enlarged (not so much that it is causing Urethra blockage), but im thinking what if my pain and symptoms are due to Non Bacterial Prostatis, and not bladder neck malfunction

    Have any of you considered Non Bacterial Prostatis?
