Bladder pain after kidney infection

I was diagnosed with a kidney infection 9 days ago. At the time of diagnosis I did not have the common urgency and pain. This began about 5 days into my antibiotic treatment and it has just been getting worse each day. I am on Bactrim DS/2 day for 10 days. Is this feeling normal? The urine culture was positive for E. coli. An ultrasound was done and indicates no kidney stones. However, there is an abundance of blood in my urine and they discovered a 5cm cyst on the kidney. Do I go back for an antibiotic extension? Has anyone ever had this pain after?

My understanding is that with a kidney infection you need a stronger antibiotic than Bactrim.  If you had no symptoms, how did they diagnose a kidney infection?

Is the blood microscopic or occult?  If occult, I'd been seeing a good Urologist asap.  When I have a UTI, my urinalysis shows microscopic blood.  Probably about 20% of UTI's do.  Occult blood is concerning and I'd be seeing a Urologist asap if you have that.   Is the cyst a simple cyst or complex?   You need to know that because a complex cyst can be serious.    I do not think it is normal to have pain start 5 days into taking antibiotic.   Have you discussed on this with your doctor?

Hi, sorry i wasn't clear i didnt have.the uti symptoms of urgency and pain. I had the severe flank pain, high fever, nausea, etc. I didn't notice the blood, which the Dr. was surprised at. I have a CT on Monday to look more at the cyst and am being referred to a urologist after. Unfortunately we just moved so our Dr. is in another city.

I had a cyst on my ultrasound as well.  Got the CT scan and it was a simple one and the rest was ok too.

Is the blood visible without a microscope?  Is your urine reddish?

I think you need to be on a stronger antibiotic, but I'm no doctor.  Bactrim never worked for me even with a simple UTI.

Another thing, and this is probably irrelevant, but when I started taking Bactrim, I got very ill with it.  I got a fever and nausea but no flank pain.  I was scared and went to emergency and they admitted me for one and one half days on observation.  That probably isn't the cause of your nausea, but the Bactrim definitely can cause it and a lot of people are allergic to the sulfa in Bactrim.
