Blind End Syndrome of Colon due to a past surgery

I learned recently that 30 cm of my colon ends as a result of surgery that I had 22 years ago. It functions similar to a diverticuli except it is a very long "pocket" and has caused me serious issues over the years. It can be corrected with surgery. The alternative is to take antibiotics the rest of my life. But I would like to learn more by having a comprehensive health assessment to see how this disorder is affecting my health as I'm very curious if it has caused or contributed to my declining memory and arthritis. I live in Ontario Canada. Can anyone recommend a clinic that specializes in gut health and has Gastroenterologists as well as registered dieticians?
