Blotchy red rash/irritated skin on penile head (lasted for over 3 moths now)?

Part of the skin on the head of my penis has become slightly red and is causing some pain/irritation.

It first appeared the day after I used a condom so I think I must have had a reaction to the latex or some other substance in the condom.

The tip of the penis became super sensitive and noticeably hurt whenever it touched my underwear.

The skin was and still is noticeably red close to and directly around the meatus. This redness gets worse and bigger after I've come out the shower or masturbated.

An hour or so after ejaculation, I get a slight burn/irritation at the very end of the urethra when I urinate and this can last for a few days before subsiding. This has put me off from masturbating as much now.

This has lasted for about 3 months now with no signs of improvement.

I saw a doctor and was prescribed timodine/hydrocortisone cream and this didn't have much effect and am slightly worried it may just make things worse so have stopped using it for now.

It almost seems as if the redness comes from within the skin rather than on the surface.

I don't think its balanitis as I haven't had that for over 12 years when I was a kid and that was with the usual smegma discharge etc, whereas this is just redness. 

I've attached images to show how it looks straight after showering, which is when the redness is at its worst and the contrast between the normal skin colour and the redness can be clearly seen.

    I've been following with this post for half a year now with the same described symptoms from a similar situation and I have a completely different theory. Fordyce, It usually targets the lips but rarely can go to the tip of the penis. It would explain all the negative test results and weird rash. Doctors still don't know the cause of Fordyce but it is definitely can be related to sex. 

    Personally this issue happened to me after a sexual encounter. I noticed Fordyce on my lips and soon had burning sensation after urination and started to get the rash on the penis tip. Soon after I felt Prostatitis symptoms. I believe the Prostatitis  is also caused by the Fordyce moving internally. Again this is just my theory.

    Now my symptoms are manageable. The head still has the rash but the discomfort is minimal. The Prostatitis sensation is this there but used to at pain level 7 and is now at a 1 even 0 at times. I believe time will take that pain away completely but if I get another Fordyce flare up it can cause issues again.

    The good is if it is Fordyce it can be removed from the skin however I believe it would be really painful with laser.

    Just to add I did see a dermatologist, urologist and they couldn't figure it out. Tested for all stds and uti and all came back negative. Ive been on antibiotics for the last 6 months, used corticosteroids, antifungal hydrocortisone cream and clotrimazole.

    I found best treatment occasional vaseline. Any strong creams just makes things worse and aggravates.

      There are so many bacteria that are not being cultured or considered to be an STI.

      Has anyone tested if you have h. pylori or some other bacteria which usually lives inside digestive system and is difficult to kill with ordinary abx dosages. Or bacteria which is spread by ticks; borrelia, babesia, bartonella and so on. If you go to Lyme disease forums, you'll see that it can be related to UTIs and any other imaginable symptom.

      ?And no one ever checks for viruses, unless you have obvious lesions or warts.

      ?There can be also some allergic reaction to something that started living inside urethra or toxins that they release when die.

    Like everyone else in this thread, I have similar symptoms.  It took me awhile to figure out the cause and effect, but now I am certain of what's causing the redness/splotches on the glans.  After ejaculation, the splotches will appear...at times almost immediately.  The more frequent the ejaculation, the worse the redness.  I have contacted a GP, dermatologist and now a few urologists.  I've had diagnosis from prostatitis, urethritis, balanitis, etc.  I recently was tested for gonorrhea/chlamydia and was negative.

    I've had HSV since 2008 but these symptoms haven't come on until last August (2017).  I don't think they are sex-related, but right after I started dating someone I started getting the symptoms.  It originally was told it was a rare yeast infection, but the treatment didn't seem to do much...then it developed into what they thought was belanitis...I applied the cream they recommended but eventually it just went away.  It could have been just because I stopped having sex.  Regardless, I know I am not offering much help here except it's nice for us all the know it isn't in our heads.  I am disappointed in the medical community that they don't seem to have much of a clue as to what the remedy is for something that appears to be more commonplace that I first thought.  I think it is good for us to continue to share our experiences so as to maybe help rule out what isn't working for us and what is.  I will pledge to offer any updates.

      you say are certain what is causing it so what is causing it? hsv? your post is confusing.

      I am finding that there is a direct correlation between ejaculation and the red splotches/sensitivity on my glans. Sorry for any confusion.

    So I came to this forum several months ago for the same reason as everyone else here. And I wasn't going to post unless I made some progress, so here's my story. 

    My irritation started a year ago March of 2017, when I noticed a burning redness on the right side of my foreskin. At first I thought it was chafing from working out, or vigorous masturbation, so for two weeks I did nothing and tried to be "delicate", and it just got more painful. So I went to my GP, two urgent care physicians, and a walk-in clinic over the course of the next three weeks desperate for someone to give me a quick easy cure. None of them could confirm anything solid by looking at it. So I was told to try some over the counter anti-fungals since I've been prone to jock itch in the past.

    For two months I treated it meticulously, and it seems to improve some, but then would get worse and better with seemingly no pattern. So one of the family practitioners suggested cortisone cream, and I tried that, but it seemed to make it spread, and I ended up with splotchy red marks on my glans too after using it, but there wasn't any discomfort with those red marks. The burning and inflamed skin on the right side stayed though 

    Finally in May of 2017, I saw a dermatologist, and she told me it looked like dermatits. (non-speciific irritation). Great! So she prescribed another cortisone cream and told me to call back if it didn't clear up. I applied the cortisone, and it felt better but was even more red. So I convinced myself she didn't know I had a fungal infection and got more religious with the Lotrimin. Besides, it wasn't bothering me that much, and if I slept nude, and wore loose boxers I thought it was getting better. 

    I'll jump forward to February of 2018.So I'd spent the last 9 months applying anti-fungal (Lotrimin) every day, and would look at my penis at least 20 times a day, convincing myself it was mostly better - and sometimes it was!. Then while on vacation, I'd spent a day on the beach and in the rain, and for some reason it became irrited like it was day one all over again, and stayed painful for the rest of vacation. 

    I'd had enough, so I called the original dermatologist and went back. Fortunately she cultured the affected area, and two days later reported her findings. She said that the culture showed Klebsiella pnumonia growing, which is a normal bacteria found in the gut, but I had it as a skin infection on my penis. She treated me with an anti-biotic for the bacteria, and then prescribed Hydrocortisone Butyrate for the inflamed skin. And I cleareed up quite a bit, but not 100%

    So when I went back for the follow up appointment, I saw the practices founding dermatologis (30+ years expeience) for a second opinion, and he said that it was contact dermatitist (something I'm coming in contact with). I asked him if the bacterial infection had caused the dermatitist? (they can be mutually exclusive evidently) And he said that it was likely the other way around. The dermatitis allowed the bacteria to settle in to the weakened skin. 

    So here's the real cure. Contact dermatitis kept sticking in my head, and I noticed that the rash was the best every morning when I got out of bed after sleeping nude. No pain, and no redness. Then  as the day wore on, I could feel it a little and it would get more irritated. I also noticed that I felt best nude in general, or wearing these lightweight cotton pajamas around the house. Anything else I wore seemed to irritate it. All year, I thought the rash was made worse with heat, so I spent most of my time at home letting things breath so to speak. 

    Bottom line, it was my underwear. I switched to 100% cotton underwear from the cotton blends, and polyester blends I used to wear. No redness, No pain. No drugs or creams anymore. Today is March 19th of 2018, and my culture and treatment started on February 14th. I can still feel it a little after sex, but it no longer looks red and irritated at all. 

    I'll keep my fingers crossed, but the dermatologist told me that many skin conditions don't really cure - that I may have to live with it. He said that It may come and go throughout my life. What they (dermatologists) hope to do is get these skin conditions into remission, and keep them there. Many can't be cured completely. 

    I hope that helps some of you. I was just as desperate as most of you are to solve this. And I'm sure my condition was a bit unique to me. But keep seeing the professionals until someone can help you.

      Really great post mate much appreciated, your experiences sound exactly like mine, but I'm running out of things to try so definitely going to look into changing my boxers now! The majority of people I've seen suggested contact dermo but I thought I'd eliminated everything..

    After reading blckshep68 comment I've done some more reading and it's very interesting. I found this link which has some useful comments and also a link to a very comprehensive skin diagnosis website. Well worth a look and I am now definitely pursuing the idea of a synthetic allergy of some sorts!

    Is it possible... that OP developed penile psoriasis following an initial HPV infection?

    As the body naturally fights HPV, psoriasis can be triggered from the response of one's immune system. The chronic inflammation of the HPV lesion(s) is what supposedly causes this:

    PS: A topical chemotherapy drug called 5-FU is an effective nuclear option for those who are really struggling w an agressive HPV. I am happy to share more details in a follow-up post.

    I've had this redness for about 5 years now I think... When I first noticed it my glans were bright red. I thought it may have been overrubbing and not using lube when I masturbated.

    Looking back i think it was a reaction to latex in a condom as I remember it happened shortly after. It could also have been to my poor diet at the time. I wasn't conscious of what I was eating and drinking at all...

    Anyway, first i would try all sorts of creams... any old skin moisturizer thinking it would quickly clear up... Then a few prescribed. But nothing worked... The redness always appears less first thing in the morning, more after a shower... More after a long day at work.... 

    I then tried to change my diet... I quit sugar and lost a lot of weight dropping a few stone. I drink a lot of water. I still eat a lot of bread and drink coffee. Seems alcohol (red wine and beer) can make it look worse.

    I'm trying a new cream called 'childs farm light moisturiser'. Look it up. Been mentioned in the UK news recently as this miracle cream for eczema. After 2 days it does seem to be less... i dunno. i will see after a week.

    I've been reading about certain foods to avoid if you're also allergic to latex... Foods like bananas and tomatoes... Both of which I eat everyday. I'm sure it's something that I eat still or drink. It could be bread or coffee...hard to give either of those up

    Maybe people should list food/drinks they have regularly or everyday.

    Mine would be: bread, butter, chicken, tomatoes, cheese, coffee(3-4 cups), milk, oranges, bananas,

    I will update how it goes after a week with the Childs farm cream.

    So much info to take on but really comforting to hear so many people in a similar boat albeit very frustrating and hope everyone finds a resolve.  I'm 34, circumsized and my red irritation (5 years now) looks more like a tiny cluster of blisters on the glans that flare when I masturbate or have sex / have an erection.  Flaccid you can't really see anything unusual & for me it's more of a vanity thing - I'm married (my wife has never mentioned anything), fairly fit, eat and drink ok & they certainly don't hurt me.... it just bothers me to look at!

    I have tried hydro cortisone and sudocreme but they didn't seem to do anything.  I find it really hard to keep the applications up when I see no progress.  One guy I read said to use a scourer to scrape off the skin and get new skin which I actually did but they just bled and returned - quite extreme and don't advise!

    Can I ask, and this might be ludicrous, but do health professionals you have seen ever ask if you can get an erection to demonstrate the issue at its most prominent state?

    I am really fussy about my boxer shorts so am going to interrogate that.  I've also considered using talcum powder to keep the area dry but not sure if that will help.

    I have seen a doctor about a complaint I had in my pelvic area and testicles but he said there's nothing unusual and again nothing unusual about my penis in its flaccid state... I have been tested for STDs and come back clear. I sometime gets bleeding gums so don't know if something is going on orally.

    Could be a yeast infection, could be my boxers, my diet, a former partner I don't know...

    It's taken some courage to even write about this and sorry if it's not the exact same as what you're going through but this thread is the closest I have come across.

    I thought I'd go whole hog and upload an image.  Maybe someone can help me more than the docs?!

      Hey mate takes a lot of courage putting yourself out there,  we are all in the same boat so no judgement here. To be honest after the 3rd or 4th doctor fiddling with your junk you kinda just forget about the nerves! I've never heard of a doc examining an erect penis but I'm sure it must happen. Nearly all of the docs I've seen have put minimal effort into examining my problem brushing it off as mild when the discomfort and physical change to me is much more than that. Very frustrating.  My issue is the opposite, looks much worse when flaccid and kind all blends in when erect although I do get similar bump on the tip of my gland. I also get bleeding gums, don't know if that's related. I feel like I'm showing all the symptoms of a yeast overgrowth although I've now taken medication for that and changed my diet etc and have yet to see any change.  I've just got my new cotton boxers today, I've definitely been wearing polyester the whole time I've had the issue so there's a glimmer of hope there is some kind of allergy. I'm pushing my current doc for more substantial testing next month so will keep everyone posted!

      hello, there may be a violation of the metabolism of vitamins (the folate cycle may be disturbed or the metabolism of biotin). such a photo is not very informative

      Hi Aboyko - thanks for the info. What should I do? Take Folic Acid? Also how can I provide better pics? More of them? The biggest flare up comes from sexual activity.
