Botox for overactive bladder/nocturia/frequency/urgency

My bladder wakes me up frequently at night and I am making 5-7 trips to the bathroom. During the day I can last about 1.5 hours before seeking a bathroom. I had a PAE with minor success, but now my IPSS score is back to around 30. My urologist is suggesting Botox injections into the bladder to control this. He doesn't think a TURP will help me. Has anyone here had this Botox injection procedure and if so, does it work? Side effects?

    does the constant urination affect the colour of your urine ? i only ask as mine is always very yellow

      My urine color is a bit darker than it should be, but if I drink too much liquid I am peeing all the time.

    I take Tamsulosin (Flomax), and it does help. PAE is widely discussed in these forums - Prostate Artery Embolization. The IPSS is a question and answer test that asks about one's ability to urinate, including frequency and urgency. A score of 30 is very high and indicates a lot of difficulty. My score is always around 29-32, so I have a lot of trouble emptying my bladder and lots of discomfort.

      With the number of times you are urinating, are you sure you don't have a UTI (it might be mild). Since I had a recent, excruciatingly painful UTI, I have been checking my urine with urine dipsticks. I bought a brand off of Amazon. I then bought the Siemens brand off of eBay. The Siemens brand indicates that the cheaper sticks bought off of Amazon lack quality control. If you decide to get some, buy the brands from the large, reliable companies (Siemens, Roche). They cost more, but to me it is well worth it.

      I like the idea of testing my urine with dipsticks. I will get some. A couple of months ago I believed I had a mild UTI and went to the lab for a urine analysis. Came up negative. However, I began taking high doses of vitamin C for a couple of days and the bladder irritation improved. This came up a few days ago, and after two days on vitamin C I feel better this morning. So, some of the urge to pee might be from a low level UTI. My last PVR ultrasound showed I am retaining 170ml of urine after I think I have cleared my bladder so there may be some bacteria "stuck" in the bladder that is causing some issues.

      Other things to try to help prevent UTIs, esp. if they are caused by EColi:

      1. D-Mannose
      2. Cranberry pills
      3. Cranberry juice as long as it is pure with no added sugar or anything else (preferably organic to avoid pesticide residues)
      4. A product called biofilm defense. Check it out on Amazon and read the reviews. Check out the company's website as well. From the company's website, it works best when taken with a substance that kills bacteria (antibiotic, garlic, oregano, monolaurin, etc.). I don't have direct quantitative proof but since I have been taking it with monolaurin and oregano oil, I feel like my bladder has improved. You really need to read up on biofilms if you want to try this and read those Amazon reviews.

      What was the name, make, or model number of Siemens test sticks you used ? Do you know the name or number of any other good sticks from Roche or any other companies, and where to get them ?

      Siemens Multistix 10 SG, purchased on eBay for $20 for 100 (I'd forgotten that this is a good price).

      I haven't tried the Roche sticks yet but saw a video of a nurse describing how to use urine sticks. She didn't mention the name of the dipsticks that she demonstrated, but I could see the label at various times and deduced that the model number is Chemstrip 10MD. However, I just looked on eBay and can only find the Chemstrip 10 with SG (which I believe means specific gravity). The strips are Roche or Roche Cobas (I'm not sure if Cobas is the name of a company that Roche took over or merged with). However, Roche is a large Swiss pharmaceutical/diagnostics company and I would feel confident with them. Cost is $60 for 100 strips.

      Even though I have the Siemens dipsticks, I'm going to buy the Roche sticks as well. Urine dipsticks are not infallible so I want to have two different sticks to evaluate my urine.

    I would try Myrbetriq before Botox. It has a very low side effect profile and is often very effective on overactive bladder.

    None of the treatments for BPH (Flomax /PAE) will help overactive bladder and compression socks won't either. I would try meds first and if they don't help enough, try Botox, but only with a doc with lots of experience with it on bladders.

      I have been studying the Botox procedure and came to the same conclusion as you. I am currently taking Trospium, similar to Myrbetriq. It does help a bit. However, my urologist is not sure if another treatment for BPH (such as TURP) will help me or not. I disagree with him.

      My last PVR showed I am retaining 170ml of urine after voiding. Add 100ml of urine per hour at night, and my bladder quickly fills and wakes me up every 1.5 hours. I pee (2ml/sec - very slow flow), then wait a few minutes, then pee again getting a bit more out. Then back to bed. About 1.5 hours later, I am up again to repeat the same process. My urologist thinks the problem is with my bladder, but I am not so sure about this. He never did a urodynamics test. I think I am retaining too much in my bladder and if I could really clear it then this might buy me more time at night between bathroom trips. The same thing happens during the day.

    I'm reading your posts with interest. Almost I went to the GP as i felt I had an over active bladder. Yes I did have, but I also had prostate cancer!

    Before radiotherapy I had laser surgery to 'open things up'.

    Radiotherapy followed and this resulted in seeing off the cancer (last PSA test was so low that it couldn't be given a number.)

    This has made the OAB worse (radiation cystitis)

    Consultant now suggests cleaning up the scar tissue and bladder slough, then botox.

    I'm very concerned about retention and self catheterisation. I had to do this once a week before, I LOATH the idea of doing several times a day over a long period.

    Anyone any thoughts?

    I had the botox treatment and it worked for me I was using myrbetriq but it wasn't working that well. First thing is to treat the blockage then deal with the bladder Good luck
