BPH - Urinary Hesitancy: All the time, or mostly just overnight?

For those of you with BPH and difficulty initiating urination, do you have that problem every time you try to void, or mostly just overnight when you wake up and have to go? It typically takes me 10-20 minutes (sometimes longer) to initiate urination when I wake up overnight, but I usually can go within 30 seconds during the day and within 2 minutes in the evening. My urologist says I have a "difficult case" with overactive bladder, pelvic floor dysfunction, and BPH (even though he says my prostate is a normal size for my age (55) and I have a PSA under 0.5), and he has repeatedly encouraged me to consider UroLift. But if BPH is the source of my difficulty initiating urination, I guess I don't understand why the prostate is generally only causing this problem overnight and not throughout the day. I don't want to have a procedure on my prostate unless I'm confident that the prostate is actually the source of my problem.

    mine is pretty consistent but the hesitation does vary depending on how full my bladder gets. at night it gets a bit more since i can sleep 2 hours before needing to wake and daytime have to go every hour

    Do you mean you have difficulty starting and a weak urine stream?

      Difficulty starting urination only. I would not describe my urine stream as weak, though it is not as strong as it was when I was younger (I also void about 14 times per day on average, which I imagine reduces the power of the stream vs. holding more urine in and going less frequently). My stream is also never interrupted once it starts, although sometimes if I wait a bit I can release an additional spurt or two.

    My symptoms are definitely worse when I wake up at night. (primarily much weaker stream and longer time to empty). I too have wondered why this is so. I'm mostly normal during day. I have virtually no hesitancy starting during the day, 5-10 seconds at night.

    My symptoms followed the same general pattern as yours, although not as severe. For example, I only had a limited number of times when it took me 10 minutes to start urinating. Usually the problem would just be a very weak flow. The problems generally started in the evening when I camped out in front of the TV, usually after 10:00 p.m. My theory was that once I stopped moving about and exercising, the muscles sort of fell asleep and no longer worked efficiently.

    For what it is worth, I had Aquablation and this problem has been eliminated for me.

    I also have difficulty getting started the first time I wake up at night, and not complete emptying ... have to get up a few minutes later to go again. No problem like this during the day or when I wake up again in the night. I asked my uro doc about this, and I couldn't understand his answer, but bottom line seemed to be it could be a few different things, and it's complicated.
