Breath that smells like penicilin - possible cause?

Hi,

my son's breath during the night and in the morning smells like penicillin (best comparison I can come up with). He doesn't take any medication apart from Fe supplement because of the low Fe levels in blood.

Any ideas/thoughts on what might be the possible cause?
