Bumps Near Anus

so this is a jumble of issues and I'm just trying to figure out what is going on. Just bare with me, I have been dealing with an external hemorrhoid for about 6 months, I can't remember when it started and about 3 months ago I got covid. after getting over covid I noticed my throat was sore and continued like this for a few months. I didn't really pay attention to any of this until I had sex with someone and 2 weeks later started having breathing issues along with bowel movement issues. the person said he was clean and I still got the full std panel testing. All were negative, it's been a month since then, my throat is getting better but 4 days I've noticed my hemorrhoid is agitated and when I took a picture I saw these little white bumps. I can't tell if they are hair follicles or something else but there is a rash present. I have no been eating the best or staying hydrated and have also been straining to use the bathroom. long story short I just want to know if these bumps are something to be concerned about.
