Have you ever tried cryotherapy? It is where your stand inside a chamber that fills with cold liquid nitrogen. Pain relief is one of the main objectives of cryotherapy- it helps to relieve soreness, muscle pain, and stiff joints. It is used long-term for chronic conditions such as back pain as well, so it seems it would fit your needs. Cryotherapy even burns calories each session, from about 500-800 per use. It is becoming a much more common treatment for athletic injuries and soreness/ pain after workouts, yet it also shows improvement for chronic conditions. It is very safe, but there are certain situations and people where it will not fit, so I would suggest asking a doctor if you are interested in the therapy.
