Can Epididymitis come back after doxycycline treatment?

My brother was diagnosed with epididymitis about 3 months ago and after 2 weeks of  doxycycline (1 pill a day) from the urologist the pain in his right testicle was completely gone. 

After one week of pain free, a mild, more of a aching soreness came back on the right testicle and also a mild shooting pain by his right leg (not sure if it's related, but he did go to the gym the week he felt better) 

Does epididymitis just take a while to heal, or is this a recurrence?

    Both it can take time to heal and once it has healed it can back at any time
