chills, fatigue and painful urination/ejaculation, normal culture, PSA, ultrasound

Background: Healthy 30 year old male with office job, regular physicals that return normal results, never had unprotected sx, regular masturbtion (once a day).
Symptoms: I have been suffering from (in order of first appearing symptoms) dull and sometime stabbing left flank pain that sometimes travels to butt, burning tip and stabbing abdominal pain after urination specially in the mornings, burning tip and scrotum after ejaculation, fatigue and chills.
1st Month: At first a walk in clinic doctor claimed UTI and gave me Cipro AB for 3 weeks along with extensive organ ultrasounds, several specific urine cultures, blood PSA and Gonorrhea/Chlamydia tests. By the time I finished the course all tests returned normal. Cipro solved abdominal pains but nothing else.
2nd Month: My family doctor claimed I have complicated prostate Infection that won't show up on any culture tests and prescribed Doxy for 3 more weeks. I finished the Doxy course but it did not alleviate any symptoms, although it did reduced the pain level. I feel like Doxy exasperated fatigue, chills and flank pain.
3rd/This Month: Finally got in to see a Urologist who diagnosed me with none bacterial Prostatitis, and prescribed 500mg of daily Quercetin and physio therapy. So far none have helped.
Question: I'm failing to understand why would I have chills, flank pain and fatigue if this is not an infection? My physical therapist and chiropractor have so far failed to help with any symptoms with stretches and exercises for CPPS. To this day peeing is a painful journey every time, specially when I wake up. flow is normal. any help or suggestion is appreciated.

My understanding is that the only way to be sure you don't have bacterial prostatitis is to culture semen. I have 3 uros not one of them would do this. Used to be when uro did DRE and touched my prostate it hurt a lot. I ended up with a UTI and was on bactrim for 2 weekI asked my GP for another 2 weeks and she gave it to me. I haven't had muchtrouble since. I still have BPH problems but no pain
