Chronic Bartholin Abscess and Cysts - My Experience

After roughly 5 years of experience with these beasts I opted to have my gland removed. I feel as though I've been through hell and back and almost feel a sense of obligation to share my story to those who may be going through the same thing. I know I wanted to know everything about them, so the least I can do is contribute my knowledge of the topic.

*I am NOT a medical professional and I am not dishing out medical advice, just sharing an experience*

I got my first abscess towards the end of my senior year of college. I'm now 27. I had the abscess drained by OB and it did not reappear until several years later. And guess when it decided to return? My honeymoon. My honeymoon was completely ruined. We were in the Dominican Republic and I noticed it starting to swell up the morning after we arrived. By that afternoon I was already starting to feel sluggish and almost like I had the flu. By the third day I was in bed with a 102 degree fever. I was too embarrassed and nervous to see an actual doctor in the Dominican so we went to the resort clinic and I faked a sinus infection knowing they would give me antibiotics. The antibiotics did not help (cephalexin) and I still had several days until I could get a flight back to the states. I writhed in pain for days, I didn't sleep for 3 days straight. I made a half-assed attempt at popping it WITH A NEEDLE. That is what desperation will do to you! The pain was so bad I had to stop and LAY DOWN IN THE AIRPORT when we were trying to get home. Do you know how disgusting and humiliating it is to lay down on the ground in an airport? I refused to get help until we got home. There is just something about getting cut open by someone you don't know in an unfamiliar place that is just unsettling...I cried for the entire day during the course of our 2 flights home. My husband rushed me to the ER as soon as we landed. The doctor lanced the abscess and gave me a word catheter. I felt amazing afterwards. It hurt to sit, but I could work from home and within a week I was back to normal. Story should end there with a happy ending, right?

Wrong.

Fast forward to a few months later and it's back. I tried the sitz baths, but it did not change. When it got big enough and I started to feel that awful flu-ish feeling, I saw my OB GYN who once again lanced it and inserted a catheter. Went home, did my sitz baths, back to normal in a week...

Fast foward another two months and I feel another one coming on. Once again, I got the catheter. Except this time my OB stated she made a larger incision than before. It hurt extremely bad this time. I cried at the pharmacy waiting for my pain meds and cephalexin. I almost couldn't make the drive home. That night, I got chills and noticed I was bleeding way more than normal. I also developed another high fever. By that time I was a veteran with the word catheter so I knew something was wrong. Me and my husband called local hospitals trying to figure out how to classify "too much blood loss". I toughed it out and tried to sleep through the night. I was sure it was infected and that it was bleeding too much. 3 days of bleeding with chills and fever, puss burst through the catheter, almost as if it had re-burst again after it was lanced. It was a horrible, horrible experience.

Later that winter it came back once again...like clockwork, I swear. This time I had a marsupilaization. The procedure seemed almost TOO good to be true. I felt way better afterwards than when I had the catheters. We even went out to eat that night! This blissful feeling lasted for 3 whole months! I know it seems odd, but when my husband and I were intimate after the incision healed and the stitches dissolved, it was better than it was before this whole mess started!

Too good to be true, you say? You are correct.

Here we are...summertime, a little over a year after my honeymoon disaster. On a holiday weekend I felt it coming back. I went and saw my OB and she said she would lance and insert a catheter. At that point, I finally took control of what I thought should be done and declined the catheter. I requested a lancing instead because at that point - I already know it will come back, so why go through the extra pain of the catheter? She lances it, it got infected (despite the antibiotics) and re-burst again a few days later. Same fever, same pain, no sleeping, etc. I cried once again, but this time not just because it hurt, but because I felt broken. I felt like it had officially taken over my life.

Before I left my OB's office the day I got it lanced again, we talked about a more permanent solution. I mentioned removing the gland entirely (because like you people, I troll the internet looking for all possible solutions to this thing) and she shrugged her shoulders and gave me an uneasy look suggesting that removing the gland was not something she would advise. 

The fact that she wouldn't even hear it out made me feel a bit less comfortable with her even though she had been my OB for 14 years! I once again took to google and searched for the BEST gynecological surgeon in my area. I settled on one and set an appointment for a second opinion. My visit with her was borderline inspiring. She was so confident, she expained that I was considered an ideal candidate for the surgery and even went so far as to draw pictures of the actual procedure for me. It sounds weird, I know, but it helped. She sent me on my way and said to call back when I was ready. I called back two months later when I just barely felt another abscess coming on and she put me on the surgery calendar. I met with her the night before and even sat down with the anesthesiologist to get a very clear picture of how everything the next day would run. The surgery went great. I'm 3 weeks post-op and feeling really well. There is still a little pain, but definitely no bleeding. I'm used to healing really quickly so I keep expecting it to not hurt at all, but that is just unrealistic considering a gland was removed from my body. 

I'm not going to tell anyone on here to go out and do the surgery. It is up to you.

Below is what did not work for me. 

Sitz baths

Tea tree oil

Epsom salts

Silicia

Serrapeptase

Probiotics (these are good for you anyway)

Drawing salves

Massaging the area

Steam/warm compresses

Ice packs

Antibiotics

Calamine lotion

Here is a little procedure cheat-sheet for those who may be unfamiliar:

Lancing - the area is numbed with a topical anesthetic, again with local anesthetic and a small incision is made into the abscess/cyst to allow the built up puss to drain. **soooo gross**

Word Catheter - incision is made similiar to the above, except a small tube with a balloon on the end is inserted into the hole. This allows the hole to remain open and encourages the cyst to continue to drain. My first doctor said the catheter should stay in place for a few days. The internet told me 4 weeks. My second doctor that performed my excision also said it should have stayed in for at least a few weeks.

Marsupilization - incision is made per the above and instead stitched open to allow the hole to remain open. From what I've heard, this has been a permanent solution for some! Not for me, but I definitely preferred it over the catheter.

Gland Excision - exactly that. The entire gland is removed. Recovery time is around 6 weeks, pain is mild, I was also warned that disfigurement is a possibility, but that was not the case for me. Yes, I've looked at it. 

It may be too early to tell if my surgery has a good prognosis, however, my doctor did say she was able to remove almost all of the gland. It is important to know that the gland is so small and the area is so vascular, that it can sometimes be difficult for the surgeon to remove the entire mechanism. Understanding how much was removed will help you better figure out your chance for reccurrence. 

Well friends, I wish everyone on here the best of luck with this awful condition. It blows my mind that this disorder doesn't get a whole lot of attention considering how life altering it really can be. I leave you with just a few of my mantras for battling this crap: you are not alone, you are not broken, and you are not crazy. Do your research, don't be afraid to get second opinions and do what you feel is best for your body and your well being.

    I'm so glad I took the time to read all the posts here so have got right up to date and heard your brilliant news!

    It's great to hear that you have done so well post-op and it sounds as if this has been a permanent solution for you. Fantastic! Thank you for sharing - I'm researching gland removal and it's so good to hear a success story.

    Hello Megan, I'm really glad to hear that someone has gone through the removal of the Bartholin gland and had a positive outback. I'm sad to say that I'm one of those who had a negative one. I've had the removal surgery a year ago. After just two months the supposedly ''removed'' gland on the left side started to swell again and I was in big pain and really sad, crying all the time. After that all doctors told me that they won't do anything on me, because they don't know what my doctor did actually. My doctor acts stupid, saying that he did what he could. He literally did everything wrong. anyway, I have reoccuring swellings every month. I am fighting it with better healthier food and no stress environment. I lance it myself because I'm sick of doctors. I don't know where I get the bravery to do so, but I've practiced it for a dozen times and nothing went wrong (I make sure everything is sterile). I don't know what else to do. I'm nervous and angry, I see no way out. Please help me with an advice. Any kind of advice.

      I'm sorry to hear things are not going well for you. I would do some deep research to find another doctor in your area that can give you a second look. Specifically one that has experience in the gland excision. Even if they don't know what your doctor did, they should still be able to provide other solutions based on your current issues. I know you say you make sure everything is sterile, but I would definitely still see a doctor for your lancings. Lancing yourself is very dangerous!

      Thank you for the advice. I've contacted a well known doctor(after the surgery, I haven't really gave up on doctors like it seemed at my first post) who told me that I should see an old doctor who has a lot of experience (he didn't know for sure if he had experience with gland removals, but he said that he has experience with everything) and he told me that I would probably have to go to surgery again, surgery of total removal because he sees that a branch or something of the main gland has been left behind and it causes swellings, mainly because it has nowhere else to go? So it countinues to swell. I have really worked hard on my immunity and therefore the swelling is painless, I have no fever and no need for antibiotics. The doctor said that it just stands there and shows no signs of inflamation. I haven't listened his advice and I didn't go to surgery again, even though it's a old doctor with experience. I'm afraid that it won't work and I'm convinced that I need to clense my body from bacteria. I think that something(?) inside me causes the swellings, does that make any sense? I'm not sure that an operation will work for me, because one of many doctors I've seen for the past three years has told me that the gland cannot be fully removed and that it has branches that are complicated and can cause swellings. He mentioned that my first surgery should have been done with silver-something(I can't remember) that should be put inside the opening and burn the gland. He says that it is not painful, but I've done some research later on, and it is painful, he lied. The burning with silver is an old method that has been replaced with skillful scalpel removal of the gland. Anyway, I'm really not sure if my theory of an alternative hollistic approach will work... the total clense, healthier lifestyle and all that goes with it.

      I've mentioned that I do the lancing. I don't know, it isn't that bad as it sounds. I actually just squeeze the swelling and take an sterile needle and poke it as little as I can. Because the pressure is big the thing bursts immediatelly. When a little bit of bood comes out I know it's over. I feel no pain, I just need to rest for an hour and then I'm good to go. When the doctors did it, there would be a lloooot of blood. They would make a big opening that closed in a range of few hours. And the swelling would still occur. Do you think that I should go to surgery again(to an experienced doctor) or I should work on getting my total physical state better?

      Well, I can't say for sure what you should do as that decision needs to be entirely yours. I'm not in the medical field, so my best advice is just to carefully weigh out your options and go with your gut. This other doctor does sound like he is fairly knowledgeable. I think the branch he is referring to is the duct of the gland itself. I have heard that the duct can be blocked and cause further abscesses if not removed when the gland is removed. The silver is colloidal silver. I'm not familiar with the specifics of it, but I've heard the term used in conjunction with bartholin related procedures.

    Your story is pretty much identical to my own! It's horrible to know that so many women have this problem! But I am a little relieved to know that I'm not alone! I am due another abscess any day now! I say this because another cyst has appeared over the last week and I'm so used to them now I know what to expect! I've had the cysts since I was about fifteen and I'm now 24 they only started turning into abscesses over the past maybe three-four years! I've had them lanced and drained so many times I've lost count, I've had two catheters and had the marsupilization twice but still my gynae will not even think about removing the gland. At my last appointment he said that he didn't feel comfortable doing it as its a very difficult surgery and it will ruin my sex life (due to dryness) and leave me very scarred he said I will probably grow out of them!! I just don't know what to. I know it sounds silly but when I feel a cyst I sit there and cry because I'm so fed up of these evil things taking over my life! I had an abscess about a month after my first marsupilization and because I still hasn't healed properly from the op it just tore my skin open and burst througg my poor boyfriend was with me at the time and we had only been together a few months but luckily he has been very understanding!

    Has anyone ever had luck with any homeopathic remedies? I'm in the process of booking an appt with my local homeopathy doc because I feel it's my only option. Should I try and find another gynae who will be willing to remove the gland! I just don't know anymore and any advice would be very much appreciated!

    Hi Megan,

    You poor thing for all the things you have been through! I am so sorry you had to live through all that and am happy to hear that you are finally releived from this horrific monster. I, myself, am a sufferer from repetivite and AGRESSIVE bartholin cyst abscesses. Each time I get one I resort to google for "therapy" to see that I am not crazy for being in so much pain and, more importantly, am not the only one. I finally decided to write about my experience as well so I can hopefully contribute to someone else's comfort as I have been comforted from these discussions. 

    I moved to Istanbul from American for an "adventure"/ learning about my culture about 5 years ago and have been here ever since. It seems as a present to my move, my body decided to create a huge and painful lump on my vagina in addition to a more difficult life than I was living in the comfort of Texas. Finding the best doctor here, I immediately scheduled an appointment. He said he will removed the cyst and I went on to have the surgery. After 2 weeks of finally feeling relief it returned! I though it was the doctor's fault. He told me that I must be the case of returning cysts and recommended I have my gland removed. I was FREAKED OUT. I thought maybe he just wanted money and I made the biggest mistake of my life by not trusting his words. I said I would think about it and meanwhile, I took antibiotics and the cyst disappeared. 

    About 6 months later, it returned with a vengence! this time, I went to the American hospital here, in excruciating pain. The doctor said he would marsupulize it and releive my pain. The pain of that procedure was the worst thing I have ever experienced and I dont think the local anestethia he gave did a thing because I felt EVERTHING. Although it did really releive my pain for the day. I would not recommed this at all. Not only was I screaming for him to finish quickly the whole time, my legs were literally convulsing after the whole thing and I had to lay there for about 10 minutes to come to myself again.

    3 weeks later, once I finally healed...guess who decided to pay me a visit again..my good ole buddy Bartholin cysts! Yes, cysts as they came up on both sides this time. I found a doctor at the American hospital who had spent time working in the US. As soon as I met him, I loved him! He spoke to me in English, explaining everything (as I was used to being treated like a patient rather than a human before). He removed some of the abscess and put me on antibiotics to help clear the infection until my operation. He told me he would remove all my cysts and we scheduled an appointment for the operation the following week. I urged him to remove the glands as well as I did not want to live through another day of this horrid thing.

    Once the operation was done, he said he was able to remove all my cysts but wasn't sure if he got the gland because of how much of a "mess" it was. He said I had about 4 cysts on my right side and the left side had 2. 

    The recovery was awful, but eventually I healed completely. And ladies...your vagina does go back to normal, dont worry that your woman hood is disfigured. once you heal, its as good as before.

    Now, 9 months later, the cyst came back with a vengence (seems to come back worse and worse each time). Am I so lucky that my doctor would be in Paris for the next 3 days? Bingo! I literally was in labor for 72 hours with this thing, spread eagle, half naked. Thankfully, my fiance (who after researching and reading your story, Megan) seemed to finally understand that I am not overexaggerating the pain. Poor guy literally took such good care of me, trying to keep my spirits up the whole time. I was finally able to see the doctor after 2 days (he saw me earlier than anticipated as I directly contacted him and said I couldn't take it any longer). Being on the third floor of  a non-elevator apartment, my biggest concern now was how the heck I was going to go down all those stairs, get in a cab and get to the doctor with this Franken-vagina. I carefully got ready I mentally prepare myself for the worst pain of my life. The stairs seemed easiear than I anticipated. I was surprised. Strangely I felt a warmth coming over my vagina. Then the smell...Oh goodness the smell. I asked my fiance if he farted. He said no. Then I put these all together: warmth, no pain, horrid smell....I reached my hand between the baggy skirt I was wearing to discover a bloody mess on my finger. I literally ran up those 3 flights of stairs, feeling everything running down my legs, my fiance following me with gagging sounds. Imagine if I had been in the cab when that happened!

    I washed off and made it to the doctor. The cab ride was a breeze as now, I was so much more relieved. The doctor saw me and perscibed me a whole regimine of antibiotics, antibiotic cream, ice pack placements and anti-inflamatory medications. I have an appointment in a week to talk about surgery. and by the way, the cysts burst in two places...on my labia majora...thats right..on my skin! No wonder I was in horrid pain. He said I have the worst case of resilient Bartholin cysts he has ever seen in his 30 years as a doctor.

    I dont prefer going under general anesthia 2 times per year/ 4 weeks of healing, so I am going to beg him to remove my glands this time. I hope this time it will be gone for good. I hope I have a success story such as yourself. 

    I appreciate anyone who has taken the time to read this, and I'm sure we all have similar horrid stories of this monster, just know yours is not the only one. All I have to say is men are lucky and girl power! 

    Megan, my final question to you is...how are you post op? Your last update was over a year after the operation and you said you've been doing great. how about now? Also, can I learn the name of your doctor? and where she is? I would love to possibly see her. I am willing to do anything!

      I am still doing well. No recurrences or complications. My husband and I are expecting our first child, which we're very excited about. Has your doctor mentioned if he is willing to do the excision? It sounds like he's seen a lot of bartholins cases, which is definitely a good sign. Many don't even know what they are!

      He said he has seen cases of bartholin cysts but has not seen anything as resilient as mine in his career. So, I am not sure if he has ever excised the actual gland before. I will see him on Monday. I am thinking whether I should ask him to refer me to an oncologist if he hasn't before...I've been reading that oncologists have more experience. Was your doctor an oncologist who performed the excision on you? My doctor is a obstetrician/gynocologist, so I'm afraid if he even leaves a tiny bit that the cysts will return. and this time, making it more difficulty because of scar tissue/ the fact that it will be too tiny for the doctor to remove later.

      Thank you! My doctor was an obgyn. I've actually never heard of an oncologist doing the excision since only in rare cases are bartholins cancerous.

      I see, ok good to know because I was starting to get worried that an obgyn wasn't qualified to do it.

      I have an appointment with another doctor in about a week. I just had a quick question...which imaging technique did your doctor use to see whether the bartholin gland was completely removed? No doctor seems to say there's a way to see. Maybe I need to teach them wink

      Hello!

      You know, that is a great question but I'm not actually sure! I received a call after the procedure stating the pathology results said my surgeon was able to get nearly 100% of the gland and duct, but I don't recall them stating how they knew that. I think at that point I was just too relieved to care to ask myself!

      Already 4 months since I had my bartholin gland removed. They will know if completely removed through biopsy result. I'm a medical Technologist so I know. Post-op Infection is common one reason is that we cant avoid urine to touch the wound. Bacterial and candida albican yeast infection may occur. You'll know by requesting a vaginal or wound culture laboratory test. Im ok rightnow but still having pain and more white discharge specially during menstruation due to hormonal changes during mestruation and so ph change too that ph change may cause an imbalance between the beneficial lactobacillus bacteria present in vagina and assist in the overgrowth of Candida albicans.       Thank you so much xtria! I have supposedly had both my bartholin glands removed, but after suffering another abscess I'm starting to wonder whether the doctor was able to remove all of the glands. He claims I have "chronic inflammation" which is unlikely since it felt and looked just like a bartholin cyst abscess.

      What do they take a biopsy of exactly?
