chronic pain in scrotum, leg, and foot and scrotum feels weird

i have had an ongoing condition going back at least 7 years now, with my left testicle initially at times poking out in odd directions momentarily and also becoming swolen and red and causing pain off and on. i tried various antibiotics to no avail. this continued until the pain spread to the right side of my scrotum and the pain has been chronic ever since, for about 5 years or so. the pain also radiates down my thigh and even has entered my foot now too. it has also caused significant testicular atrophy and has lowered my testosterone levels. something i've noticed is that the skin and the stuff inside tends to shift around on its own continuously. the skin also tends to go leathery and hard, especially when i touch it. the veins on the right side in particular seem like a jumbled mess when i touch it and they're kind of everywhere, covering the testicle where they didn't used to. they also can get very swolen. sometimes i can touch or see the swelling and theres a big lump, i'm not sure if its cords that have gotten twisted or what. they also seem to tighten and cause the testicles to rise up at times. i also have to pee very frequently now and it gets urgent feeling really fast. sorry if i'm rambling, the pain is pretty sharp right now which urged me to make this thread and it's difficult to concentrate. it seems like touching them and trying to massage them just makes things worse. i'm pretty desperate but don't know what to do as doctors just say there's nothing wrong as ultrasounds never pick anything unusual up apparently. could it be torsion, varicocele, a combination of both? i don't really know but a varicocele seems likely at this point as the veins on the right side have become like i said twisted and often swolen. hopefully i've left enough hints for someone to at least have a suggestion as to what could be wrong.. but i really fear like it's too late at this point. what are left of my testicles feel really deformed and again it feels like a complete mess around them with the tangled cords covering them and potentially some abscess too. it's worth noting that pressure tends to build up when i lie down and sometimes when i wake up they're in a crazy amount of pain because of it. i guess there's a problem with the blood flow and this has caused the shrinkage. anyway i hope someone can help in some way, i don't know what to do and i'm getting pretty desperate as its getting to the point where i'm less and less able to just ignore the pain and i can't enjoy or focus on anything. pain relief medication and anti-inflammatory pills seem to have little to no effect. hot baths don't really seem to help either.

I have pain from my testis causing tight hip flexors, adductors, psoas and piriformis, which all perpetuate pelvic pain and pain in my leg and foot on that side. Everything hurts less when I massage these muscles out. Especially my psoas, deep in my abdomen. i really feel like these muscles can effect testicle sensations, as well as the pelvic floor. My testis are very sensitive to touch, but i don't have the swelling issues that you have.
i would really try to find a pelvic health specialist. Someone needs to take a look at the whole picture and consider your quality of life. Testis don't atrophy for no reason.
Don't lose hope my friend, you're not in this alone. You know your pain the best, you've just gotta communicate this to the right doctor and get the help you need. Much love.
