Cognitive impairment starting at 16?

Hello all, i've been through a lot of testing and would appreciate everyone's opinion on my symptoms. They first started when i was 16 i noticed twitching all throughout my body followed by muscle weakness and speech problems and swallowing issues to the point where i couldn't swallow my saliva.(The swallowing issues went away the speech problems are off and on). Anyways these aren't even the symptoms that give me the most difficulty. When all this started i noticed my memory was weaker than it was previously. However, i paid no mind to it after doctors reassured me i was fine and attributed all my symptoms to depression/anxiety. And yes i did get severely depressed but that was after i noticed my symptoms. I started on SSRI's and noticed that my memory progressively became worse over the past 3 years. Now it's at the point where i could forget something i literally just did. For example i'd move something in my room and i would immediately forget that i moved it.(This happens like 10 times a day) I struggle to recall conversations that occurred 15 minutes ago. I know this isn't caused by anxiety/depression because i'm currently taking medication for them and none of this worries me anymore it's normal to me at this point. I've noticed my memory getting weaker and weaker over the years. I had a CT scan 4 years ago when i first noticed problems and just recently a few days ago. The doctors told me that everything's fine and normal. However when i look at the scan from back then and now my brain looks completely different? Also i have episodes of confusion and my multitasking abilities are getting progressively worse. Any thoughts and opinions are welcome.
