Dermoid Cyst in my Right Ovary

So the beginning of this week I went to my gyno since I have been bleeding for the last 4 weeks. She did a transvaginal sonogram. She discovered that I have a pretty big fibroid in my uterus which is causing me to bleed. But to our surprise she also found a 2cm cyst on my right ovary. When she told me this I almost hit the floor. I'm pre-menopausal, 47. So my Dr reassured me that she didn't think it was anything serious but she wanted to run blood tests to confirm and also send my scans to two Oncology specialists for them to review independently. So yesterday I get a phone call from her yesterday telling me that both specialists have confirmed that it looks like a dermoid cysts, which kind of makes sense since my mom had the same thing on the same ovary. My Dr proceeded  to tell me that she wants to see how the blood tests results come back and then schedule to remove the cyst, my ovary and Fallopian tube in that side, and remove the fibroid at the same time. My question is this, does anyone know is it possible for an oncologist specialist determine if a cyst can be dermoid from a scan of this nature?

    No, they can't 100%, that's why it needs to be removed and worked up histologically, which is the only way to see 100% what it is made of,

    but see, when 'growth' was found in my 10cm cyst, I was told by the looks of the ultrasound, that it looked malignous and it turned out histologically (even to the naked eye due to hair, bone) to be a benign dermoid cyst....

    MRI scans and doppler ultrasound (looking at the vascularity) can help giving an estimate of 'risk' and 'likelyhood' and if they think it was a dermoid most likely, it will most likely be.

    Where are you based? When my cyst was first discovered the local sonographer could only see that there was a thickening in the cyst which was quite scary. I was lucky enough to get a referral to UCH in London where They have high-tech scanning equipment. It was clear using their equipment that it was a dermoid - I saw all the hairs on the scan!

    I have not had mine removed but if or when I do the UCH consultant advised tube as well.

      Im in the US in New Jersey. I know there are dinstinct characteristics of dermoid cysts. I'm anxiously holding on with hope that two different cancer specialists said the same thing when evaluating the scans. I know it will be confirmed once removed. I'm a VERY anxious person and reading everything on the internet I think makes it worse! I'm also dealing with a rather large fibroid in my uterus whic has made me bleed for over a month now. My iron is low and having all this anxiety is not helping. I can't eat or sleep and cannot wait to have it removed. Thank you for responding.

    Hi! Yes, they determined that for me with my MRI scan and ultrasounds I had done. They can tell by the shape and the fluid / solid parts etc...

      Thank you Jackie for replying I have been a wreck! Can't sleep can't eat it's horrible! I think the large fibroid is causing many issues for me as well. I just thank god I went for the scan and they caught this otherwise I would have never known! Did you have yours removed? My Dr wants me to have mine removed along with the ovary and tube on that side. My mom had the exact same thing in the same side but hers was the size of a baseball!

      Yes. I had both ovaries and both tubes removed along with 2 cysts, both on my right ovary. The larger one was 10cm and the other was 2cm. Best thing I ever did! 

      I opted to have the left ovary removed at the same time because I am 52, had started Menopause and it was more for preventative reasons. I didn’t want to go thru surgery on one side and then later have cysts start growing on my left. I was having open surgery and only wanted to go thru that once! Now it’s one less thing I need to worry about!

      I agree, I will talk to my Dr to see what she says I really do not want her I have to go through this again it's been so stressful. Can I ask how the surgery was? Your recovery? How long after did you start to feel better? Sorry for all the questions 😊

      No problem, open surgery went very well, no issues. 

      Recovery smooth however I have to say  it was more painful than I expected but they give you pain meds And you just need to keep taking them and not let the pain get ahead of you. I would be off work for about 6 weeks (I don’t work) I found it uncomfortable to sit and you literally cannot lift anything for min 6 weeks - I did it longer

      ....i’m now At the 3 month mark and feeling good! Just starting to exercise in small does again etc...takes a while for the internal stitches to heal. 

      No regrets, would do it again!

      If you have keyhom or laprascopic surgery it will be a breeze! My cyst was too big so it was open surgery

      Thank you Jackie so much! Yes it will be Laproscopuc for me. Can I ask you to what types of symptoms did you have? I have lower back pain, a little bit of swelling not much, very emotional, and nauseous most Morning.  I was told that with these types of cysts since the characteristics have human cells in them they resemble symptoms of being pregnant, did you experience any of this? Oh yeah anxiety through the roof'

      Lucky you, it will be much easier for you from what I have heard! 

      My pain was a dull ache that built as the day went in. By night i had to lie flat, very uncomfortable! The pain moved around as well, to my hip and groin area. Very bloated - I looked pregnant by nighttime! Had to pee all the time!!! Constipated. A bit anxious too!

    The waiting is the worst part, and reading things on the internet will only make it worse.  I know, as I read them.  The ultrasounds and CT scans can provide a good indication of what it is expected to be.  In my case both said indicative of dermoid.  My blood work and tumor markers were normal.  I began as an emergency case, thinking it was appendicitis.  However, they didn't 100 percent know until a) the frozen samples tested during the procedure (removal of one ovary, and two tubes) and b) the final detailed pathology results that came about a week later.  I seemed more concerned about a negative result, than the docs.  Surgery was about 4 weeks ago -- keyhole.  Recovery wasn't automatic, but each week over all has been a better week.

    Try to take a deep breath and not get ahead of the results and the procedure.  I know, easier said than done.  The odds are with you, and no amount of stressing on your part will change anything -- except have a negative impact on you.  These are the things I would tell myself.

      Thank you EdanaGrace. I'm just afraid of my blood test CA125 test will come back elevated due to my fibroid and all the bleeeding. I'm worried sick. Have you heard of this happening? I have done so much research in this CA125 and how inaccurate it is especially in pre menopausal women.

      The CA125 test as a diagnostic tool can be inaccurate.  There is a question about the accuracy of the other markers as well in diagnosis.  They are indicators.  Don't fret about a blood test -- again easy to say.  Heck, when my bloods came back normal I was still fretting with the what if scenarios.  It did nothing but stress me out.  Just remember the odds are with you in this, and save your worry for something concrete to worry about should that ever happen.  Also if there is a way you can help move things along to get in quicker that is always a good thing.  I am lucky as it was just a couple of weeks for me.. a long couple of weeks..but in retrospect so much quicker than I see for others.
