Hi S, A picture would help but it could be a pigmentation thing you have going on, try seeing a dermatologist for a real answer. OK

Sanjay, this sounds very much like Lichen Sclerosus (LS) / Balanitis Xerotica Obliterans (BXO) - although this is becoming a more common condition, many GP's / Doctors are not familiar with symptoms and what to treat it with.  I had my LS / BXO diagnosed 7+ years ago and my foreskin is healthier now that it has ever been.

So, STOP applying anything to your penis, and washing it with soap / shower gel etc.  The ONLY thing you should be applying is a Steroidal ointment - and this should give you a positive improvement.  In time the tightness / itching / white patches will improve.  The typical treatment steroid is Clobetasol.

If you are in the UK you can either go back to the GP and ask to be referred to a Dermatologist or Urologist.  Alternatively, go to a specialist Sexual Health Service - and no, the condition has nothing to do with whether you are sexually active or not.  Say you think you have LS / BXO and they may want to conduct a biopsy to prove exactly what it is you have.  The good thing about a Sexual Health Service is they should recognise your symptoms (which your Doctor clearly has not) alone, and start you on a course of Steroidal ointment then and there.

Once your skin starts to recover, you may need to start simple foreskin stretches routinely.  This will give you back the elasticity in your foreskin.  Be very wary of a medical professional who suggests you need a Circumcision as this is totally unnecessary .... Once you have started your Steroid treatment you can update your discussion forum here, if you want any other information on this ......

It looks like your LS affects a larger proportion of your foreskin than mine.  Are you able to get to a Medical Professional / Sexual Health Service to get the right help?

hi its the same , the faded skin will continue to get thinner.  I was using steriod creams for a year , its help with the itching and burning but does not health the underling cause. for me it got to the point when i could not roll the foreskin back . its a skin issues as per the Urologist , it has no cure.. and sex was difficult since when the penis is erection the tightness of the foreskin lock off the penis .

Hi Guy, I have been dealing with foreskin issues since the age of around 16.  My LS / BXO was diagnosed 7+ years ago - I have only ever used Clobetasol to treat it AND my foreskin is in better shape now, and I gain greater sexual satisfaction because of that fact, than I have ever done before throughout my life.

I do have to stretch my foreskin, really, on a daily basis.  I use 2 main simple stretches, drawing the foreskin away from the body, and, drawing the entire skin of the shaft of the penis towards the body.  I have developed the personal knowledge of just the right amount of pressure I can now use, and, the amount of repetitions I need to do to keep my foreskin healthy.

So, are you placing anything on your penis regularly, such as lube, or condoms, even soap, shower gel / lotions, skin lotions ointment, washing powder can ALL have a severe impact on exacerbating the LS and can be the cause of keeping LS in a flare-up situation?

What have you tried to help minimise the impact of your LS?

Just on the subject of the "skin getting thinner" - if you are using a Steroidal ointment that is suitable for your skin (most LS sufferers use Clobetasol, which is the recommended treatment) the role of the steroid is to keep the skin as healthy as possible.

To achieve this, it has to be applied correctly, and, applied frequently enough to suit the individual sufferer.

Where scarring affects LS affected skin, the role of the steroid is to thin the skin so that it is as near to being healthy as possible, again.  If the steroid is NOT applied correctly, potentially too much steroid can over-thin LS affected areas, AND, if the steroid is applied to HEALTHY skin, that is then subjected to friction, such as during sex, or self-pleasuring / masturbation such skin can then break down and bleed.

Bleeding such as this when the penis is erect can cause a lot of blood to be lost in a relatively short space of time - to deal with this, you need to loose your erection as quickly as possible, then apply an antiseptic cream, such as Germolene / Savlon / TCP to the NON-LS affected area ONLY.

    MY UPDATE:

So guys, yesterday when I went to my Urologist, he recommended me to dermatologist

Dermatologist as guessed by us diagnosed me with BXO and told me to do the biopsy. After seeing the biopsy result, we will determine either we will circumcision or not. He told me not to worry about circumcision as it will be laser circumcision.

In general terms, having circumcision to address LS / BXO is a bit like taking a sledgehammer to crack a nut.  As I have previously stated, I have been dealing with LS / BXO for 7+ years now and my foreskin is genuinely healthy now than it has ever been.  As, once a foreskin is gone, it is gone forever and you could potentially have unwanted side-effects of the surgery which you obviously don't have now.

My advice would be to try the Medically recognised prescribed treatment of Clobetasol ointment for at least 8 months before you decide to do anything else.  Should you go for the Clobetasol make sure you ask how to apply it, and how frequently.  I would recommend at least every other day for at least 3 months before you review what is going on and you may find you can reduce the frequency of application to twice a week.

One other thing I would suggest you ensure before circumcision - draw your foreskin as far down the shaft of your penis towards your body by holding the outer skin below the mid-point and pull it down as comfortably as it will go.  What I am getting at is for you to see how far the discolored flesh of the foreskin reaches down the shaft of your penis - simply, for a circumcision to be effective I would have thought all the BXO infected skin would have to be removed.

If your foreskin is not too tight, you may be very surprised as to how far down towards the body the infected area of skin can actually reach.  Something to think about .....
