Constant imbalance and difficulty focusing on objects

Unfortunately this is a tricky one that has left many stumped.

About a year ago, I came off my surfboard and felt a strange sensation from within my left ear. I didn’t take much notice but ended up getting out of the water and heading home. A few days later I noticed things felt a little strange and I was finding it difficult to walk in a straight line. When I would look at the horizon, it would shake up and down and when I put my finger in my ear to create a vacuum or suction, the room would shift. I saw an ENT about this who had me do CT and MRI scans (to which both showed no signs of damage). I did a series of balance/ hearing tests and the one test that showed some unusual results was the pressure test. This led the ENT to believe it was a left ear perilymph fistula. I then had surgery on that ear and the round and oval windows were patched up. However, it’s now been 6 months since surgery and I’ve noticed no change except now the tinnitus in that ear is very obvious. There was one occasion where I woke up one morning (around 2 months ago) and felt great! I did feel a little off because my brain needed to adjust but putting my finger in my ear wouldn’t make the room shift and the horizon or any object I focus on wouldn’t shake from left to right… it had completely gone. This unfortunately only lasted for about 2 weeks until one morning after a few beers at a friends birthday the night before. After that, each day got worse and worse and leaves me back where I started. I’d love to hear if anyone has experienced anything similar or has ANY advice as I'm very desperate.
