Constant vaginal bleeding and flare ups

So for the last 16 months I've been constantly bleeding (since little one was born) and most of the time it's quite heavy. In April I started having these so called flare ups (rapid heart beat, sweating, sickness diarrhea, shakes, dizzyness and passing out) I end up in hospital with this every couple of weeks but still don't know what's causing it or if the bleeding is related to the flare ups. it takes me a few days to recover after every flare but after having so many tests and doing some research I was wondering if they are even flare ups at all and just my body going in to shock? I've not got a lot of knowledge on it but is anyone able to shed some light? I am waiting for another appointment with my doctor but its 3 weeks away
