I am on 24/7 oxygen 3 lpm. I have been in several drug studies. Aside from Spiriva, I was in the FORTE study ( retinoic acid) the respimat study . I currently am on Spiriva respimat 2.5 mcg and Symbicort. My pulmo doctor recently added ARNUITY cuz mucus has invaded my airsacs.

That is really heartening. I'm 57 and was diagnosed with mild emphysema in 2015 and that had progressed to severe stage 3 by early last year, and have been aware that many people die within five years of diagnosis. I've had a number of comorbidities in the past year including arthosclerosis, gastroesophagus reflux disease, and coronary nerve blocks and suffer from constant dyspnoea and fatigue. That's basically all I have the energy to write at the moment but wanted to say your post had given me some hope I'll see 60.
