He is still crawling about the house and sometimes try to.walk on his heel as bearing any weight is.still painful for him. Could he maybe have a fracture

I am not a doctor or any sirt of medic , but 3 days seems very short to expect his foot / ankle to be free of pain, if the xay looked OK and it wasnt so swollen that they could not see , in which case they woukd have asked you to come back when the swelling went down then its probably a mild sprain, they gave you nurofen suggests there was some inflamation.

Maybe a bit of ice , keeping it elevated, a lot of rest, but not to the exclusion of keeping the joint moving even if not weight bearing - maybe a support bandage and wait 8 to 10 days, then think about going back to GP or Aand E

    Hope this helps

An XRAY we would determine if there was a fracture however it's never bad to get a second opinion. There could be a contusion of the bone which just means obviously bruising internally that could be causing the pain. It could be a hairline fracture but an MRI would be the best diagnostic test to see if there is anything else going on that the XRAY isn't able to pick up.

Yes, you want to always follow up w/ your general dr. who will refer you to an ortho.
