Could it be from the injury i had 12 years ago?

Back when i was younger i had broken my arm and my shoulder had come out of place and the doc was going to put it back into place but i refused it the doctor say i may not be able to use again but thankfully a doc change that...years passed but now its hurting i rubbed muscle rub but its still the same what should i do?

Yes it could be from your previous injury. Check with your doctor about the pain.

Definately yes. A xray or MRI would confirm. I think I hurt mine 30 years ago and it finally gave out.
