Cyst? Cholesteatoma? Something else?

I'm 55. Three years ago I started to get tinnitus. It seemed to come from the left side. I went to the doctor, she told me there was nothing to be done. I was referred for a hearing test, but it showed no impairment.

The tinnitus has increased in intensity in the last few months and out of curiosity I bought one of those otoscopes that you can connect to a mobile.

The attached image is from the left ear (ie. the one with the greatest noise). I'm wondering what this is and if it might be the cause of the tinnitus.

Does anyone have anything similar?

Thanks.
