They'll use something that is supposed to numb it. You don't feel it until it reaches the bladder to tell you the truth.

They will use a numbing a lubricating gel that is "injected" (no needle) in your urethra. The last cyctoscopy I had done was at Wake Baptist urology in Winston-Salem, NC. After the gel was applied, I was allowed to wait 15 or 20 minutes. I had absolutely no feeling of the flexible scope during the procedure. This facility has state of the art equipment with a large flat screen overhead, so I was able to clearly see what the doctor was seeing. I should say that in this case, he stopped when he encountered a stricture just before the sphincter, so he didn't pass through the sphincter or bladder mouth.
I had had previous cystoscopies with another doctor who used equipent that was definitely not state-of-the art. There was some minor discomfort, but nothing significant.
