Deeply Concerned - Please help if possible. Contains Picture

Hi,

I have been suffering from suspected Piles for the past 6 weeks, it has made my life a misery. I suspected Internal Piles however tonight I have noticed two balls pretty much blocking the anus, just after taking a siltz bath. I am concerned at how big these are, this picture is without straining.

The oily substance you see it Proctosedyl ointment but looking at the size of these I don't think it will do much,

Any advice will be appreciated, I assume these are External Hemorrhoids?image

    yes they look like external heamoroid. you can try using Anusol cream works well, can buy from pharmacy or amazon.

    hi Dan

    what symptoms are you feeling (burning, stinging, general pain, a lot discomfort during and/or after BM etc) will then determine the best way forward for them. to put it in context slightly if they are itching for example then id suggest massaging sodacream around them and that doesnt help much then have you tried germolid ointment. obviously goes without saying (if youre not already doing this) no straining when going to the loo, up your water intake, dont get constipated (get some stool softners if you are just be mindful how long you take these for and get the right type that'll help your situation!) eat more fruit and veg (make sure youre getting the correct amount of fiber in you). some general exercise can help. also if you can manage it, try to push the external hemorrhoids back up again specially if they feel like there thrombosed hemorrhoids. keep up with the siltz bath (plenty of warm water down there) but make sure its dried afterwards and obviously keep the area clean. stress and anxiety am afraid doesnt help with hemorrhoids! sorry for the long post but there is so much that can be recommended and done for these horrid things just depends on what exactly is going on down there and each person is different. happy to answer any questions you may have. stay safe and take care.

      Thanks for your reply! My symptoms have been ongoing for around 6 weeks now although I've only recently started treating them problem with ointment prescribed over the phone by the GP. Symptoms have been blood on paper, burning which is quite intense especially after a BM, discomfort when sitting and a sense of pressure that comes and goes. What's interesting is that I had very similar symptoms around 4 years ago and then again around 2 years ago, multiple Rectal exams done and nothing found. I did start to wonder if was something more serious and at one point considered a Sigmoidoscopy, looking back now I'm sure this was Internal Hemorrhoids as the symptoms were pretty much identical, the only difference being the burning after a BM which I didn't have before. I didn't think for a second that this could be External Hems due to past exams and that fact that I only had an exam 8 weeks ago, it was only because I was so fed up that I decided to look. I've read that if you have External Hems than this is a very high chance that you also have some Internal.

      I do suffer from Anxiety which is quite bad at times, it's mainly been driven over the years over symptoms relating to this, how ironic! Thank you for your suggestions I will certainly increase the fibre and continue the Siltz baths, it's quite a weight of my shoulders actually knowing what this is so that should help settle the Anxiety.

      from what youre describing it could even be fissure that you have and hence causing the stinging /burning sensation, bleeding and pressure....i would suggest you think about getting referred to a specialist but do your research first as in my sincere opinion not all surgeons are great. ive suffered with these for way over 10 years, i still bleed every time i have a BM and sometimes it stings etc... ive had 2 hemorrhoidectomys (2nd one over 15 weeks ago) ive had numerous banding (lost count how many times) botox a couple of times.... i think ive been prescribed and taken all types of creams, ointments and suppositories. ive tried all that i can think of laxatives to try and help and whilst ive always been aware that i wont be able to rid of the things i have just struggled to manage them. i get a lot of flare ups and have had to have a few colonoscopys done (all have come back clear) i have been to both numerous NHS and private drs (i personally found NHS surgeons was better than private)! basically ive realised that the more "healthy" i am physically and mentally the better i can manage my hemorrhoids and fissure (good amount of water and fiber with some exercise and really try not to think about the BM by trying not to feel anxious about the whole thing). i would deffo recommend you get a conlonoscopy done to rule anything out and the dr will also then conform exactly what i going on internally...

      Thanks for your reply, I did have a few tests done last year which included a CT scan and a Calprotectin test to rule out IBD and other Organic disease. I'm certainly going to refer to a specialist as this has been going on for too long albeit only a few times a year.

    I would advise you to see your doctor for further examination. it sounds to me that you also have internal heamoroids. There are various treatments available. I am also suffering from internal heamoroids for couple of years, with similar symptoms that you experience, iv done some changes to my diet intake and now I think I'm suffering too much so I'm going to see a surgeon next week for consultation arranged by my doctor. I just want to get to the bottom of it and get treated to move on with my life, as can be very worrying.

      Thanks for your reply. The one symptom that stood out (other than the bleeding) was just how intense the burning was, felt like petrol. This started at quite a specific time, late evening. It's a fact that Internal Hems can't be felt on DRE so it was never treated. When I first told the GP that I had the sensation of a trapped gaseous ball in the backside (like trapped wind) he looked at me like I'd just coming crashing down from the roof. I'm considering going private for the sake of £200.

    Hi Dan, hope you are doing well. I was reading your posts and it seemed like we got very close experience. This has been going on since 01/21. Couple of weeks of extreme pain down there, sudden spasms like crazy. Worked really well with Siltz bath and no sxs for a while, until 06/21 and it happened again, since warm baths already helped, this time I already know what to do, so sxs didn't last quite that long like the first one. Here comes 10/21 and pain started again, Siltz bath not anymore working, tried different kinds of anal cream, very little relief. Tried laxatives (Miralax) still not much relief. Every time Id have BM, not hurting during BM but hurt like hell few mins after and will be miserable for 2-3 hours, then after that, complete relief like as if nothing happened . Few months after that sxs still going on, gets some blood once in a while after BM, Finally consulted a gastro specialist, ordered me colonoscopy, but since I am just 32 and havent met my deductible, my COpay for the procedure was $4k . Didn't end up doing it. Now, I am taking Metamucil, better relief, there still sxs but less than how it was. I just want to have complete relief with this, makes me freak out sometimes that I may have serious condition.
