Diltiazem - skin reaction

I have been given Diltiazem ointment for a fissure which has come back after Botox in 2019. If it doesn't work I will need to have another round of Botox which is not great as I have a bit of a hospital phobia!

The problem is that while the diltiazem does seem to reduce the fissure pain, after a day or two of using it I seem to get an extreme itching and burning where I have put the cream on. It gets to the point that I struggle to sleep because it's so bad and my skin tag seems to 'swell up'

Once I stop using the cream it takes about 3-4 days for everything to calm down, but then the fissure pain returns.

The most I have managed was a couple of weeks with the cream. The question is should I try again and just put up with the skin reaction / itching / lack of sleep for the full 6-8 weeks or will this be doing more damage than it is good?
