Discharge from community mental health team

I have been asked by my CPN to think about my discharge plan so I can have good input. I am obviously anxious about discharge but know it's time. I have a brilliant GP and know that I will get good support from her.

anyone any advice for me? Good or bad. 

I really need to get this right for me and not just a corporate discharge.

    Cheers Kate.This type of discharge has happened to me as well.First to go was my psychologist.I felt a bit funny at first.But in the long run I know that if times get tough I can immediatly go back.now I can see my life and get more involved in helping others.I sit on recovery teams in the community and give group sessions.So instead of focusing on my problems all the time I can focus on how I changed things.It is more positive.I understand the seperation anxiety.Start by giving advice on what worked for you here if you wish.I know that as I talked about what worked for me to others it made my tools stronger.I even got a community star award from the health minister.So cheers and congratulations on your graduation.And so your wonderful journey begins.
