So I am in the exact same situation and have been for about 7 years. My last sexual encounter was 8 years ago, and the spots appeared 9 months later. They aren't raised and have always been asymptomatic.
I suffer from severe cystic acne and am usually on antibiotics. 6 months before the spots appeared, I developed a huge acne cyst on my cheek. In an effort to kill it, I took about 20 Solodyn (minocycline) tablets in one day.
When the spots appeared, I was confused. I noticed they disappeared completely with erection or even minor blood flow. They also seemed to disappear in a warm shower. I figured they were fungal due to my antibiotics and didn't worry too much about them. Eventually, I tried every OTC antifungal cream with no results. Thought I saw improvement with Selsun Blue, but it's so drying that I couldn't use it for long.
Last year, I had a hernia, and decided to get this taken care of as well.
Dermatologist said it might be psoriasis, might be pre-cancerous (though I've been circumcised since birth) and advised against biopsy until it developed further (which it hasn't). An online derm thought it was post inflammatory hypopigmentation (possibly from dry masturbation) and prescribed tacrolimus ointment. Neither of the derms thought it was fungal. I really believed it to be tinea versicolor, but it doesn't florescence under a black light.
Then I was sure it had to be lichen sclerosis. Tacrolimus is a treatment for that, though, with initial results in about a week. It's been a month and I've seen no improvement.
Did a full STD panel yesterday and am waiting for results. Did your test include HSV? Mine did. I've never had any blisters, sores, etc. , but I'm really at a loss. If I'm all clear, the only answer will come via biopsy.

I don't know. I don't think so because mine has not responded to the tacolimus. Have you done any tests?

Can you clarify what HSV is?
Regarding my health history, I'm circumcised and haven't had any acne issues. I had terrible acne as a teenager but once I hit 19 it all went away with a new face cleanser and I've never had anything like it since. I assumed that was just puberty / hormonal changes.
Interesting thought re: the dry masturbation as I only masturbate dry and never use lube. I didn't use lube when I started masturbating so it's just been normal to do it dry. I've tried with lube but can't orgasm. Lately tho I have been applying a small amount of coconut oil on my penis when it's soft to add some lubrication and not dry out. In general my soft penis doesn't look dry but I figured if I'm masturbating so much without lube, the coconut oil couldn't hurt. I only used it for a few days tho so I don't think its from that.
Also, I have the same issue where the spots completely disappear when I have an erection or in water. But the minute my skin is a dry, the spots show back up. Curious as to why it would be considered pre-cancerous. A biopsy on my penis sounds not fun...

HSV is herpes, but I just got my results back a few minutes ago. Completely clean for everything (10 panel test, including herpes 1 and 2). Thus, I am happy to say this is highly unlikely to be any kind of STD.
I've always masturbated dry and never had any problems until these appeared. I don't know if they're related, just a suspicion since one derm believed they were post inflammatory hypopigmentation.
The precancerous idea comes from the notion that color change is an early sign of penis cancer. Cancer is very rare in circumcised men, though, and it usually happens in ones much older years. I was 33 when the spots appeared and am 40 now.
What makes this frustrating is that the spots appear "beneath" the skin, so there's nothing to really touch or feel. I remember trying to show these to a GP when they first started and they were not visible at that time (temp / blood flow makes them disappear).

I have done all blood tests I think now it is just pegmintation from blood flow as my last derm said

i only see the discoloration when I'm soft... once I have an erection, the color looks the same all around the head.
do you see the same on your penis?

it is exactly the same when it is erected it is not noticable and when it is soft it is obvious and after i pee it appears strongly

Are you currently sexually active, or masturbating? Any discolouration along the shaft?

In a relationship and sexually active a few times a week, plus masturbating a few times a week as well. No discoloration anywhere else on my penis.

sorry no... I haven't been to the doctor yet but might go soon. It doesn't have any pain and I may be overthinking it.

Best answer is possibly. Give it time to go away, if it doesnt then its a light headed infection.

how long should i wait? i have this discoloration from 1 year and it is really making a depression for me because it looks disgusting when flacid and specially after i pee!
