Do not pull the foreskin completely

How bad is it to not pull the foreskin all the way back to clean it? I have a short frenulum and in addition I have a certain mental block in pulling the foreskin of my penis completely (childhood trauma, I think) but I don't have phimosis since when I went to the doctor, for example, he managed to pull it completely. I believe I must have smegma on the underside of my glans because I never pulled it all the way out to clean it, is that really bad? I'm 24 years old. I don't speak fluent English, so forgive me for any grammatical errors

Yes, that's just bad and not hychienic. And I'm not English either, but this kind of thing has nothing to do with language or identity. If a GP can pull the foreskin all the way back, you should be able to do it yourself, right? You should clean the glans completely every day, because bacteria have plenty of opportunities behind the glans rim this way.

Didn't the doctor advise you to make the frenulum less tight? Frenuloplasty is usually a simple procedure and very effective. GPs can do it themselves if necessary, but they usually refer to a urologist.
