Doctor has never seen anything like it... What is this?

I have had what I thought was a painless fissure for 18 months now but 3 ER Doctors said it's not a fissure and they have never seen anything like it. I have severe rectal pain, pass large amounts of mucus which occasionally has blood in it. Passing pencil thin stool, bouts of diarrhoea. This is becoming unbearable and I don't know what to do.

(Not an STI... All STI checks came back clear. Have tried Antibiotics, Nitroglycerin ointment, Sitz baths... all with no improvement. Have history of Cervical Dysplasia, including 2 x LETZ procedure and 2 years on Prednisone and Methotrexate for Undifferentiated Connective Tissue Disease.)
