Doctor Said Emergency Bladder Neck Incision Required

Hello,

My flow rate is down to 1.5 ml/s. My Qmax is just 4 ml/s. My PVR, however, is just 43 ml as of last month. My doctor said if this continues urine will back flow into my kidney and cause it to become dilated causing hydronephrosis over time. That will permanently damage my kidney, according to him. That got me scared. I'm 25 years of age. Yet to be married. I would have done that procedure in a heart beat if it were not for retro ejaculation. What are my options now?

    PVR of 43 ml should not cause I hydronephrosis. With such a low flow rate, do you need to push hard when you pee ? If you do, it may cause bladder dysfunction In the long run. Hank

      PVR of 43 ml should not cause I hydronephrosis, but pushing hard to pee might. To avoid surgeries, your best chance would be self catherization.

      I do not mind undergoing the surgery 10 years later after I'm done marrying and having kids. Do you think self catherization for 10 years is a plausible option? My urethra is awfully sensitive, which I wasn't aware of before the RGU MCU. The slightest irritation causes it to bleed.

      10 years should not be a problem once you know how to do it. Some people do it for their whole life. Since you do it yourself, it should not be as traumatic to your urethra as to when someone else does it, as in rgu mcu.

      Another option is having a suprapubic catheter installed. The only problem with this is there will be a visible catheter end sticking out of your stomach at all times. Again, some people have this catheter on all their lives.

    I'm sorry but I with you I would not do anything that would cause retro. See if he can give you a muscle relaxer. Stay away from surgery. Get a second opinion

      I took muscle relaxer and it didn't help. It was making me retain more urine and further weakened my flow.

      No problem. Do what you have to do but stay away from any surgery at your age. You are to young. You still have your future to go through.

    25 is young to do a surgery that might cause retrograde ejaculation. You would give up the possibility of having children, and have to live with it for the rest of your life.

    Have you tried Flomax (Tamsulosin) or Afluzosin to relax the prostate and increase flow rate? You can also use self catheterization, there are some here who have been doing it for many years.

    Don't let a doctor try to scare you into surgery, get more opinions.

    You should have a lot more tests before trying surgery, like TRUS (Trans Rectal Ultrsound) to determine the size of the prostate. You should have a cystoscopy to look for strictures, median lobe, and any other blockages. If you get a cystoscopy, insist on watching the monitor while it is being done, and opt for the flexible cystoscope, rather than rigid. You might also want a urodynamics test done to determine if the bladder is the problem or the prostate.

      My flow rate was 5.5 ml/s without any medicine back in March. Back then, I had no stricture. Just awfully poor flow due to bladder neck obstruction. Then, a traumatic RGU MCU test caused a slight narrowing in the mid shaft of my penis. I underwent Cystoscopy, and the doctor discovered that I had extremely elevated bladder neck. He performed Urethrotomy and slightly dilated the other side of the narrowing where catheter had struck during RGU MCU. Once the catheter was removed, my urine flow was fine for 3 days. Then, all problems started. Difficult starting urination (taking as long as 2 minutes to start), extremely weak flow rate, low flow time, high retention, nocturia more than 4 times. I took Alfuzosin for one month, and it didn't help much. I am not sure why. It feels like I have a new stricture location.

      My prostrate is, fortunately normal in size, around 10cc.

    Hi,

    Try to ask your doctor for Baclofen if it works or not. It will relax the bladder neck. If not, you have to go under surgery, unfortunately. Otherwise you will fight with yourself, and RE is not 100% sure.

    What you can do after if not working:

    • bank sperm where you can left your semen (i've done it before op)
    • go for a surgery

    Obstruction of Bladder neck is ongoing so.. no medications gonna help you sooner or later. The longer you stay with actual symptoms, the longer it will take to resolve them AND the higher is chance to f**k up bladder functions. And it's not my opinion, that is what my second doctor said. After that he said that "later on, when you will be older you will have huge problems with the bladder".
