Does it require surgery?

So, to make a long story short I had been to 2 different doctors, they were more or less dismissive of my symptoms and because my hemorrhoids are not internal they told me they will not perform surgery on me.

The symptoms are the following : Pain, pain when defecating, horrible discomfort while/after defecating, swollen hemorrhoids, hemorrhoid pain while having an erection, pain for no reason at all, pain when sitting more than 30 minutes, I have to avoid the chair that day after I have a bowel movement, pain when walking longer periods of time, sometimes pain before a bowel movement, the hemorrhoids are so sensitive and painful to touch, I feel like my guts are sticking out of myself, I feel that area all the time, it's sticking out.

They told me I am exaggerating and it's not a big deal and I should live with the pain and suck it up...They said I'm fixated on this (why wouldn't I be considering the symptoms) and I'm the kind of patient that cannot move on with his life....

I told them I am not exaggerating at all and I cannot reproduce how my butt area looks after I have a bowel movement.... I have tried so many different pills and ointments but nothing worked....

At first both of the doctors told them I need the surgery and to get all the blood work and tests done but cancelled in the last moment after the colonoscopy because they are not internal and again, no big deal to them, it didn't look like a big deal to them because I cannot reproduce how the area looks like after a bowel movement....

Please tell me I'm not going crazy with this.... Here's the picture(I wasn't even after a bowel movement in it)

    that looks so sore and no your are not causing a fuss. this is extremely painful and does take over your life.

    are u in the UK? you need to change doctor. they need steroid ointment to help with swelling for a short time. keep stool as soft as possible. i take cosmocol to help with this. drink 2 litres a day, eat load of fruit and veg. stay away from red meat and bread.

    ice the area. i use a sock! im no doc but thats what i do. put defo see another doctor.

      i am not from the UK. Other than that I have been doing what you mentioned for almost 3 years now

    what type of a doctor have you seen? i would suggest finding a good Colorectal specialist. if it has giving you so much pain for three years and impacting quality of your life then surgery might be the best option.

      general surgeon that is also competent in proctology. I know, but they refused it

    Definitely go to a ColoRectal Surgeon. That is not small and it looks very painful. At minimum I wold have them lanced
