Dry scaly, hard skin patch on penis, what could it be?

hi im 21, male and on my penis i have had a recurring problem for the past year, i have seen the doctor for it for 3 times, all times given steroid creams and they did remove the issue, but the patch always would appear, the last time it was infected and i was given a 2 in 1 anti fungal and steroid cream, it helped and didnt return for a few months, now its back, it appeared within a couple weeks, and it looks horrible, its really effecting my mental health, its making me insecure and extremely worried, i dont know what to do or if it ever will go away.
