The term "Balanitis" is a generic term for irritation on the head and/or foreskin of the penis - it provides us with no clue as to what has caused it or even how to resolve it. What was the cream the Doctor prescribed to you?

Looking at photo 1 I would say you most likely have extremely dry penile skin. Can I check, are you sexually active? If you masturbate do you always use some form of lube such as skin cream, or baby oil, that kind of thing? Also, knowing frequency of masturbation would be helpful?

You also need to think about anything that has a chemical base that is in regular contact with the skin on you penis - this includes soap and shower gel, and any other form of body wash? Do you apply any form of skin cream regularly ...... ?

my doctor gave me clotrimazole cream usp 1%. i am not sexualy active, i masterbate only 1 a day, and i often use my own spit as lube. i use mild soaps but i never apply it to my genitals. i just use warm water for it. and i dont use any skin cream often.

Clotrimazole is most commonly used for sexually active males who get a yeast / fungal infection from unprotected vaginal sex. It is possible for you to develop a yeast / fungal infection yourself, but this is most often linked to poor personal hygiene.

How long have you been using the clotrimazole?

When you take shower make sure to wash everywhere else first, then spend some time rinsing your entire groin region to remove any traces of soap, body wash such as shower gel. It is also really important to completely dry your genital skin - for this you should use a clean dry towel that only YOU use.

My concern is how your skin looks in photo one, I would suggest you would be better to see a Dermatologist - is this possible for you?
