dvt and pulmonary embolism at 41

hi, i was diagnosed with a large dvt from left calf to top of thigh on my 41st birthday.i was not admitted to hospital, just told to use heparin injections n start on warfarin.2 weeks later and i'm back in e.r. with a pulmonary embolism and an inr of 3.8. doctors cant understand how this happened while my inr was supposedly high enough to prevent more clots and cant give me any answers. would love to hear if anyone else had an experience like this and what was the cause.thank you

i am 24 and i had a DVT, pulmonary embolism at the age of 19.
I remember waking up one night to a huge cramp in my left thigh.
i left it for a couple of days until i noticed that my leg was twice its size and purple
i went to the doctors and he sent me straight to A&E
the doctors told me there that if i had left it another day than i would be dead, scary stuff!!!
i stayed in hosp for about 3 weeks and my whole life is affected by it,
my leg often hurts and gets tired,
they reckon that it might be because of my abortion or just down to the fact that i stayed in bed doing nothing while my bof was out earning the money,
i have sworn to myself that i will never be that lazy and selfish and i also have to have an injectio nevery single day during my pregancy as it passes on;
