Ectopic atrial arrhythmia on a paced heart

Hi everyone, my mother is a 90 yo with hypertension, full heart block and meditronic pacer. Recently she has been having episodes of ectopic atrial arrhythmia which have been making her very unwell. Hospitalised 3 times in 2 weeks. I have not yet spoken personally to her cardio, but he seems not overly concerned other than the negative impact on the longevity of the pacer. They are wanting to discharge her and told her to wait until her cardio appointment in two weeks which makes me uneasy. Can anyone give me any idea as to what could cause ectopic atrial beats on a paced heart? Her electrolytes were down with each episode as well.

    I'm wondering if she is developing hypokalmia which I've read can cause the ectopic arrhythmia, her k levels have been low on her blood work although not significantly.

    How long has she had her pacemaker, Medtronic have had a recall, you will need to check out if she has one that is in the recall.

    ?I notice she has 100% block, is her pacemaker a three lead, to replace the lack of Branch Bundle nerves, and or 100% BBBlock.

    I am interested as my husband has a late model Bosten Electronics, pacemaker & defibrillator, installed about 2 years ago, told it was the best available for his condition, thank goodness it has never gone off, but it does pace his heart continuously, junior Dr's who haven't seen his pacing on their monitors previously are fascinated to see it going off constantly, shows up as a little lightening bolt symbol on their monitors.

    ?We have a download of the memory of the pacemaker every six months, where they look at the battery, look at what the pacemaker has been doing, or not doing, he asked for the pace to be turned up a little a last time we visited the heart hospital, NO he was told. 

    He also has dialated cardiomyopathy to the left ventricle, so that is also echo- cardiogram each year to see if it is getting better or worse. Last time there was an improvement in his Ejection fraction, that showed up as an improvement since the Pacemaker installed.

    ?Don't know much about atrial issues other than it being another nerve that is making her heart beat out of pattern, and that you cannot leave it and hope it will go away, needs dealing with, so if they do discharge her, and it comes back go back to the hospital again, yes I know its a pain in the Butt, but hopefully you will be upgraded to a consultant that knows what he is doing, if you haven't already got one.

    What a kerfuffle to finally get him diagnosed with what was wrong, and that he had 100% branch bundle block, heartbeat at 30BPM and junior cardio's telling me it was his tablets, they removed his morning dose, and 12 hours later his heart dropped to 23BPM, at that point the hospital flying team was called in to the ward, and their advice was he MUST SEE the consultant in the morning first thing, I made sure I was there, and gave him a piece of my mind, and told him to his face his juniors were idiots. He spun on his heel, and growled at his juniors in front of us, came back himself and apologized to us, we never saw the juniors again.

    ?The biggest problem you probably have is your mums age, Dr's are going to weigh very carefully her age against improvement they can make for her with any intervention, afraid that is just a fact of life, in my Mums case, she was 87, had something seriously wrong, they never ever figured out exactly what it was, probably a lymphoma of some kind, sister who is a very senior director of nursing said we could subject her to invasive procedures, everything else had been done, MRI's Cat Scans, blood tests, etc, and or let nature take its course, she had a massive stroke 10 days later, while we were still weighing up the possibilities, Mum was included in the discussions and made it perfectly clear she did not want to be kept alive at all costs, and told us she had made a living will with end of life instructions, some years previous, which in the end had to be invoked because the Dr's wanted to keep her alive even though she was in a coma.

    ?Very hard time for you and I do understand only too well.

    Just her age alone could make her prone to atrial ectopic beats, because as we age oftentimes one or both atria can enlarge, and that can lead to irregular heart beats.

    That could occur possibly even in the presence of a pacer, I personally don't have a

    pacer but at 64 I have an enlarged left atrium and I get frequent irregularities in rhythm.  I try to ignore them, but it isn't easy.  Good luck with your mum.
